A case of localized pigmented villonodular synovitis presenting as a loose body in the medial gutter of the knee

Abstract

Pigmented villonodular synovitis is a rare monarticular proliferative disorder of the synovium, most commonly affecting the knee. There are very few reports about the localized form in the knee. A twenty-eight-year-old woman presented with complaints of pain, catching, and locking. The lesion closely resembled a loose body in the medial gutter of the knee. Magnetic resonance imaging was performed to establish the diagnosis. Arthroscopy of the knee was performed and a mass with an attachment to the medial capsule was detected. The mass was removed from the synovium by arthrotomy. Histological examination confirmed the diagnosis. During a-year follow-up, the patient remained asymptomatic. Localized pigmented villonodular synovitis may mimic other intraarticular pathologies and lead to a false diagnosis.
