Glomus tumors of the finger: a report on 60 cases

Abstract

Objectives: We evaluated patients who underwent surgery for glomus tumors of the finger. Methods: During a 23-year period, 60 patients (48 females, 12 males; mean age 34.6 years; range 5 to 60 years) underwent surgery for glomus tumors of the finger. The patients were evaluated with respect to symptoms on presentation, diagnosis, operation types, and postoperative complications. The mean follow-up was 16 months (range 6 to 72 months). Results: The most common symptom was pain (67%), which intensified when exposed to cold in 42%. Other symptoms included tenderness in 37 patients (62%), swelling in the finger tip in six patients (10%), and nail deformities in 34 patients (57%). The tumors were most frequently encountered in the fourth and fifth decades of age. Localization of the tumor was in the right hand in 32 patients (53.3%), and in the left hand in 28 patients (46.7%). The time to surgery from the onset of symptoms ranged from six months to 30 years (mean 6 years). Fifty-one patients were examined by plain radiographs, 27 of which showed concave-surface erosions in the bone; nine patients underwent magnetic resonance imaging studies besides plain radiography. The tumors were subungual in 49 patients (82%) and in the pulp in 11 patients (18%), for which transungual and midlateral approaches were applied, respectively. Postoperatively, nine patients (15%) developed recurrences, all of whom achieved clinical recovery following a subsequent operation. Conclusion: Given the considerably delayed time to diagnosis, glomus tumors should be taken into consideration in the presence of severe finger tip pain of unknown origin.

Parmak yerleşimli glomus tümörü: 60 olgunun incelenmesi

Öz

Amaç: Parmak yerleşimli glomus tümörü nedeniyle cerrahi tedavi edilen olgular değerlendirildi. Çalışma planı: Yirmi üç yıllık dönemde, parmak yerleşimli glomus tümörü saptanan 60 hasta (48 kadın, 12 erkek; ort. yaş 34.6; dağılım 5-60) ameliyat edildi. Hastalar başvuru yakınmaları, tanı konma süresi, uygulanan ameliyat türleri ve ameliyat sonrası dönemde oluşan komplikasyonlar açısından incelendi. Ortalama izlem süresi 16 ay (dağılım 6-72 ay) idi. Sonuçlar: En sık görülen semptom ağrı (%67) idi ve olguların %42’sinde soğuk ile artış göstermekteydi. Otuz yedi olguda (%62) nokta duyarlığı, altı olguda (%10) parmak ucunda şişlik, 34 olguda (%57) tırnak deformitesi saptandı. Glomus tümörü en sık 40-50’li yaşlarda görüldü. Lezyonların 32’si sağ (%53.3), 28’i sol elde (%46.7) yerleşim göstermekteydi. Yakınmaların başlaması ile ameliyat arasında geçen süre ortalama altı yıl (dağılım 3 ay-30 yıl) bulundu. Düz grafiler ile incelenen 51 olgunun 27’sinde (%53) kemik üzerinde konkav erezyon saptandı; dokuz olguda ek olarak manyetik rezonans görüntülemeye başvuruldu. Tümör yerleşimi 49 olguda (%82) tırnak altında, 11 olguda (%18) ise pulpada idi. Tırnak altı tümörleri transungual yaklaşım ile, pulpada yerleşenler midlateral yaklaşım ile ameliyat edildi. Ameliyat sonrası dönemde dokuz olguda (%15) nüks gelişti. Bu olguların hepsinde ikinci ameliyattan sonra klinik iyileşme sağlandı. Çıkarımlar: Tanıda geç kalmamak için, parmak ucundaki şiddetli ve nedeni saptanamayan ağrılı durumlarda glomus tümörü olasılığı da dikkate alınmalıdır.
