Surgical treatment of unstable fracture-dislocations of the proximal interphalangeal joint

Objectives: The purpose of this study was to evaluate the results of surgical treatment of unstable fracture-dislocations of the proximal interphalangeal (PIP) joint. Methods: The study included 13 patients (12 males, 1 female; mean age 31 years, range 23 to 48 years) with dorsal fracture-dislocations of the PIP joint. Six patients were treated with open reduction and internal fixation using 1.2-mm titanium screws within the first week of injury. In seven patients with late presentation, time from injury to surgery ranged from 21 to 56 days (mean 37 days). Of these, one was treated with mini screws, four with bone anchors, and two with mini screws and bone anchors. The mean follow-up period was 21.5 months (range 12 to 48 months). Results: Radiographic union was achieved in all the patients. The range of motion of the PIP joint was 73.4° (range 50°-90°) on the affected side, and 91.9° (range 90°-100°) on the normal side. Patients presenting early and late had significantly different range of motion of the injured fingers (84.1° and 64.2°, respectively; p<0.05). The mean flexion contracture of the injured fingers was 13° (range 0°-30°). There was no significant difference with regard to the PIP joint motion between patients treated with mini screws and soft tissue reconstruction. The mean grip strengths were 45.7 kg and 49.3 kg on the affected and normal sides, respectively. Four patients had degenerative signs in the PIP joint and one patient with late-presentation developed subluxation. Four patients complained of limitation of mild degree at work or in daily activities. Conclusion: Treatment of fracture-dislocations of the PIP joint yields successful results with mini screws in early cases or appropriate injuries, and with soft tissue reconstruction with or without mini screws in late-presenting patients or unfavorable injuries.

Proksimal interfalangeal eklemin instabil kırıklı çıkıklarının cerrahi tedavisi

Amaç: Bu çalışmada proksimal interfalangeal (PİF) eklemi ilgilendiren kırıklı çıkıklarda cerrahi tedavi sonuçları değerlendirildi. Çalışma planı: Proksimal interfalangeal eklem dorsal kırıklı çıkığı nedeniyle tedavi edilen 13 hasta (12 erkek, 1 kadın; ort. yaş 31; dağılım 23-48) değerlendirildi. Altı hastaya yaralanmanın ilk haftasında mini fragman için 1.2 mm titanyum vida ile açık redüksiyon internal fiksasyon uygulandı. Geç dönemde başvuran yedi hastanın birine mini vida, dördüne kemik çapası, ikisine mini vida ile birlikte kemik çapası uygulandı. Geç dönemde başvuran hastaların tedavisi yaralanmadan sonra ortalama 37. günde (dağılım 21-56 gün) yapıldı. Ortalama takip süresi 21.5 ay (dağılım 12-48 ay) idi. Sonuçlar: Tüm hastalarda radyografik kaynama sağlandı. Son kontrollerde, ameliyat edilen parmakta PİF eklem hareket aralığı (EHA) ortalama 73.4° (dağılım 50°-90°), sağlam tarafta 91.9° (dağılım 90°-100°) ölçüldü. Erken ve geç başvuran hastalar arasında ortalama EHA açısından anlamlı fark görüldü (sırasıyla 84.1° ve 64.2°; p<0.05). Ameliyat edilen parmaklarda ortalama fleksiyon kontraktürü 13° (dağılım 0°-30°) bulundu. Mini vida ve yumuşak doku rekonstrüksiyonu uygulanan hastalar arasında PİF EHA açısından anlamlı fark bulunmadı. El kavrama kuvveti ameliyat edilen tarafta ortalama 45.7 kg, sağlam tarafta 49.3 kg ölçüldü. Dört hastada PİF eklemde dejeneratif bulgular saptandı. Geç başvuran bir hastada PİF eklemde subluksasyon gelişti. Dört hasta işte veya gündelik yaşantısında hafif düzeyde kısıtlılıktan yakındı. Çıkarımlar: Proksimal interfalangeal eklem kırıklı çıkıklarında erken ameliyat edilen veya elverişli olgularda mini vida ile, geç ameliyat edilen veya elverişsiz kırıklarda mini vidaya ek olarak ya da tek başına uygulanan yumuşak doku rekonstrüksiyonu ile başarılı sonuç alınabilir.
