A case of a rnultiple enchondromatosis and hemangioma (The Maffucci-Kast Syndrome)

The Maffucci-Kast syndrome which is very rarely encountered clinically is characterized by multiple enchondromatosis and hemangiomatosis. Commencing with the first identification of this syndrome. In 1881, there have been only 105 cases reported in the world literature all together until 1973. We have been able to evidence this disease both radiographically and histologically under our observation conducted in a girl 5,5 years of age who presented with muitiple bone affections and a localized hemangioma. Due to a severe genu valgum position, a corrective osteotomy was performed in the proksimal one-third of the tibia in this case, where the tumor significantiy metastasized over the skeletal system. As the plane of osteotomy could not be relied upon as regards to its osseous weight-bearing capability even 9 months after the clinical operation, we have equipped this child with a suppiementary walking splint. On the other hand the fibula which was cut in the meantime while the corrective osteotomy was being performer has promptiy attained healing. It is apparentiy seen here that osteogenesis has not been disturbed in unafected regions of bone in the Maffucci-Kast syndrome

BİR MULTİPL ENKONDROMATOZIS VE HEMANJİOM OLGUSU (MAFFUCCI-KAST SENDROMU)

Klinikte çok ender olarak ortaya çıkan MAFFUCCI-KAST sendromu mültipl enkondromatozis ve hemanjiornatozisle karakterize olur. Bu sendromun 1881 yılındaki ilk tanımlanmasından sonra 1973 yılına kadar bütün dünya literatüründe yalnız 105 olgu yayınlanmıştır. Multipl kemik afeksiyonları ve bir hemanjiom lokalizasyonu gösteren 5,5 yaşındaki bir kız çocuğundaki gözlemimizle biz bu hastalığı, hem röntgenolojik ve hem de histolojik olarak kanıtlayabildik. İleri derecedeki bir genu valgum duruşu nedeniyle bu olguda tibianın üçte bir proksimalinde, tümörün kemik sistemi üzerinde önemli bir ölçüde yayılma gösterdiği yerde, bir düzeltme osteotomisi yapıldı. Klinik olarak operasyondan 9 ay sonra bile osteotomi düzleminde osser bir yüklenebilme sağlamlığı olmadığından, bu çocuğu bir yürüme destek ateti ile donattık. Ama düzeltme osteotomisi yapılırken aynı zamanda kesilmiş olan fibula, osser bir iyileşmeye çabucak ulaşmıştır. MAFFUCCI-KAST sendromunda, hastalığa yakalanmamış kemik bölgelerinde, osteogenezisın bozulmadığı burada açıklıkla görülmektedir.
