Our experiences on diagnostic arthroscopy of shoulder
Abstract
27 patients with operative indications from which 18 suffered from instability and 9 from impingement syndrome were examined arthroscopically from March 1994 through April 1996. Our goals in using this method were: reassuring the indications for operation and finding other pathologies. All operations were carried out under general anesthesia by the same surgeon. We also used lateral decubitus position with 60¡-65¡ of abduction, 15¡-20¡ of anterior flexion, 4 kg traction and standard anterior and posterior portals. From cases with shoulder instability, 16 had Hill-Sachs lesions, 14 Bankart lesions, 6 fraying of subscapular muscle, 3 degenerative labral tears, 1 SLAP-3 lesion, 4 biceps synovitis, 14 synovitis and 1 loose body. From impingement cases 4 had biceps synovitis, 4 rotator cuff fraying, 7 synovitis, 5 degenerative labral tears, 2 complete rotator cuff tears, 1 SLAP-2 lesion and 4 fraying of subscapularis muscle. No complications were seen in relation to application of arthroscopy. Etiology of shoulder instability, glenohumeral pathologies in impingement syndromes and the ability to discriminate these two entities are among the most important advantages of diagnostic arthroscopy. Nowadays the popularity of shoulder arthroscopy is because it helps us to make the correct diagnosis, to check the preoperative diagnosis and to use it as a treatment method. Even though including few cases this study is considered a promising field for our clinic in future.
