Surgical treatment of lateral discoid meniscus lesion in adults

Abstract

The results of 17 adult patients with symptomatic lateral discoid meniscal lesions were retrospectively analyzed. Six of the patients were female, mean age was 25 (19-42), mean follow-up was 31 (12-47) months. Six subtotal and 11 partial meniscectomies were performed. A patient with a Wrisberg type lateral meniscus underwent saucerization and postero-lateral repair. At the last follow-up visit, 10 cases (59%) were graded excellent, 5 cases (29%) were graded good and 2 cases (12%) were graded fair using the Ikeuchi scale. Mean Lysholm-II score was 90 ± 7 (75-100). The Lysholm scores of patients undergoing subtotal meniscectomy were slightly better than those undergoing partial meniscectomy, but the difference was not statistically significant. No correlation was found between Lysholm scores and age or Lysholm scores and length of follow-up. Good results can be obtained in patients with lateral discoid mensical lesions if a partial or subtotal meniscectomy is performed taking into consideration the type of tear and meniscal morphology. Saucerization and posterolateral repair should be attempted in Wrisberg type lesions if certain prerequisites are met.

Öz

Semptomatik lateral diskoid menisküs lezyonu olan ve cerrahi tedavi uygulanan, 6'sı kadın 17 olgunun canlıdan değerlendirildi. Hastaların ortalama yaşı 25 (19-42) olarak hesaplandı, ortalama izlem süresi (12-47) aydı. Altı olguya subroral, 10 olguya parsiyel menisektomi yapıldı. Wrisberg tipi diskoid menisküsü olan bir hastaya parsiyel menisektomi sonrası postero-lateral tamir uygulandı. Son kontrolde, Ikeuchi skalasına göre 10 olguda ('% 59) çok iyi, 5 olguda (%29) iyi ve 2 olguda (% 12) orta sonuç elde edildi. Ortalama Lysholm II skoru 90 ± 7 (75-100) olarak bulundu. Subtotal veya parsiyel menisektomi yapılan olguların Lysholm II skorları karşılaştırıldığında, subtotal menisektomi yapılanların skorları biraz daha iyi bulundu, ancak bu fark istatistiksel olarak anlamlı değildi. Hastaların yaşı ve izlem süresi ile Lysholm skoru arasında anlamlı bir korelasyon bulunamadı. Diskoid lateral menisküs lezyonlarında yırtığın tipi ve menisküsün morfolojisine göre parsiyel veya subtotal menisektomi ile erken dönemde başarılı sonuçlar elde edilebilece-ği, Wrisberg tipi menisküslerde, gerekli ön şartlar yerine geliyorsa, saucerizasyon ile birlikle posterior tamirin uygun olacağı kanısına varıldı.
