Our results of revision total knee arthroplasty

From 1987 to 1997, thirty-one revision total knee arthhroplasties (TKA) performed in 30 patients with failed primary TKA were studied to determine the short-term clinical and roentgenographic results. There were 29 females and 1 male. The mean age was 65.19 years and the mean follow-up time was 22.17 months. The reasons for revision arthroplasty included infection in 17 knees, aseptic loosening in 7 knees, extensor mechanism dysfunction in 3 knees, severe instability in 3 knees and periprosthetic fracture in 1 knee. The outcome of the TKA was assessed using the Knee Society clinical and roentgenographic rating system. The knee score improved from a preoperative value of 52.61 points to 82.36 points and the function score improved from 31.96 points to 61.20 points at last evaluation. Preoperatively the mean maximum flexion was 78.4º and improved to an average of 90.8º at final follow-up. There have been 7 reoperation (22.5%) and 5 recurrent infection (16.1%) after the revisions. Especially one-stage procedures had recurrence of infection. The other complications were one heterotopic ossification (GI), two patellar tendon rupture, one dislocation. One of our patient died of sepsis 20 days after reimplantation. The results suggest that predictable and satisfactory results can be obtained in a revision TKA. We concluded that revision total knee arthroplasty using a systematic and standart approach will have infequent complications and will provide a satisfactory result inselected patients.
