Proximal femoral osteotomy for Perthes disease

Öz

Proximal femoral osteotomy for Perthes disease was performed on 16 patients in Ministry of Health Ankara Hospital 2nd Orthopaedic Clinic between October 93 & December 95. Average age at the time of operation was 7.9 (range 4-12), male to female ratio was 13/3. 13 patients that had a minimum follow-up of 6 months were evaluated. Functional and radiologic evaluation was performed both pre and post operatively. Functional evaluation consisted of ROM, leg length discrepancy and subjective complaints. Radiologic evaluation consisted of Catterall evaluation of femoral head, epiphyseal in extrusion index and measurements of center edge angle of Wiberg. Average follow up was 21,3 months (range 8-34 mo). As a result we can say that best results with proximal femoral osteotomy can be obtained in patients without any femoral head deformity flattening, with good containment in abduction & internal rotation and children between the ages of 6-10 years.

Sağlık Bakanlığı Ankara Hastanesi II. Ortopedi ve Travmatoloji Kliniğinde Eylül 1993-Aralık 1995 tarihleri arasında 16 Pertehs’li hastaya proksimal femoral varus osteotomisi ameliyatı yapıldı. Yaş ortalaması 7.9 (4-12), E/K oranı 13/3 idi. En az 6 ay takibi yapılabilen 13 hasta değerlendirildi. Olgulara preoperatif ve postoperatif olarak fonksiyonel ve radyolojik değerlendirme yapıldı. Fonksiyonel değerlendirmede kalça hareket kapasitesi, ekstremite kısalığı ve hastanın subjektif şikayetleri gözönüne alındı. Radyolojik değerlendirme ise Catterall klasifikasyonuna göre femur başı tutulum miktarı, epifiziel ekstrusion indexi ve Wiberg'in center-edge açısı ölçülerek yapıldı. Ortalama takip süresi 21, 3 (8-34) aydır. Sonuç olarak; femur başında aşırı deformasyon ve düzleşmenin olmadığı, abdüksiyon-iç rotasyonda containmentin sağlandığı, özellikle 6-10 yaş arası çocuklarda proksimal femoral varus osteotomisinin başarılı sonuç alınmaktadır.
