The role of core decompression in the treatment of avascular necrosis of the femoral head

The results of core decompression in the treatment of avascular necrosis of the femoral head (ANFH) have been evaluated. 22 hips of 15 patients, 8 of whom were male and 7 were female with an average age of 36.5 years, were evaluated. The average follow-up was 49 (min.24-max.68) months. The etiology of ANFH was steroid administration in 10 patients, alcohol abuse in 2, sickle cell anemia in 1 and idiopathic in 2 patients. Core decompression without any grafting was performed in all patients. Pre and postoperative radiologic grading was done according to Ficat's Classification and the average progression rate was %40 in Grade II hips and %87.5 in Grade III hips. Modified Merle d'Aubigne's Criteria were used for clinical evaluation and average postoperative score was 15.8. Core decompression has a very clear analgesic effect for a relatively prolonged time despite of the radiologic progression and is also a "time-buying" procedure before total joint replacement in this relatively young patient group.

Femur başı aseptik nekrozu (FBAN) tanısıyla foraj uygulanan 75 hastanın 22 kaçasıyla ilgili sonuçlar retrospektif olarak değerlendirildi. Hastalardan 8'i erkek, 7's'ı kadın, ortalama yaş ise 36.5'ti. Ortalama izlem süresi ise 49 (min. 24-max.68) aydı. FBAN etyolojisi 10 hastada steroid kullanımı, 2 hastada aşırı alkol alımı, 1 hasta-da orak hücreli anemi ve 2 hastada ise idiopatikti. Hastaların tümünde de herhangi bir greft kullanmaksızın foraj uygulandı. Operasyon öncesi ve sonrası radyolojik değerlendirme Ficat kriterlerine göre yapıldı ve Evre II'de ortalama %40. Evre III'te ise ortalama %87.5 progresyon olduğu görüldü. Klinik değerlendirme Modifiye Merle d'Aubigne Kriterleri'ne göre yapıldı. Buna göre operasyon sonrası ortalama puan 15.8 olarak bulundu. Bu çalışmadan elde edilen sonuçlar doğrultusunda forajın, radyolojik progresyon olmasına karşın, oldukça be-lirgin ve uzun süreli bir analjezik etkisi olduğu ve nispeten genç olan bu hasta grubunda total kalça artroplastisi öncesi zaman kazandırıcı bir girişim olduğu sonucuna vardık.
