The use of external fixators in the management of tibial shaft fractures

The results of treatment and complications of 58 fractures of 57 cases that external fixators (EF) were used for tibial shaft fractures between October 1986 January 1995 were evaluated retrospectively. Mean age of the cases was 29.5 (3.5 -83) years, 84.3 % were male and 15.7 % were female patients. The most common cause of fracture was traffic accident. 54 (93.1 %) of the fractures were open. EF was performed in the first 24 hour to 33 cases, and the remaining 24 case in 2-30 days. The cases were followed-up for approximately 13 months and early amputation was applied to 3 patients. For 55 fractures of 54 patients, the duration of external fixation was 130 (55-240) days and healing period was 168.2 (80-275) days. Of 55 fractures, 9 (16.3 %) non-unions, 3 (5.4 %) wound infections and 5 (9 %) pin-tract infections were observed. Also, Sudeck's atrophy was present in 5.4 %, drop foot in 10.9 %.

Ekim 1986 Ocak 1995 yılları arasında tibia cisim kırıkları nedeni ile eksternal fiksatör (EF) uygulanan ve izlemeleri yapılan 57 olgunun 58 kırığı, tedavi sonuçtan ve komplikasyonları açısından retrospektif olarak değerlendirildi. Olguların yaş ortalaması 29.5 (3.5 - 83) yıl olup % 84.3'ü erkek. %l5.7'si kadın idi. Kırık ne-denleri arasında trafik kazası birinci sırada iken, kırıkların 54'ü (%93.1) açık, 4'ü (%6.9) kapalı kırık idi. 33 olguya ilk 24 saatte, 24 olguya yaralanmadan sonraki 2-30 günler arasında EF uygulandı. Ortalama 13 ay izlenen olguların 3'üne erken dönemde amputasyon yapıldı. Gen kalan 54 olgunun 55 kırığında EF uygu-lama süresi 130 (55-240) gün, iyileşme süresi 168.2 (80-275) gün olarak saptandı. Ellibeş kırıktan 9 (%16.3)'unda kaynama yokluğu, 3'ünde (%5.4) tedavi ile sonuç alınan yara yeri ve 5 (% 9)'inde çivi yolu infeksiyonu gözlendi. Ayrıca olguların % 5.4'ünde Sudeck atrofisi, % 10.9'unda düşük ayak, % 3.6'sında dizde ve % 20'sinde ayak bileği eklemlerinde kısıtlılık saptandı. Elde edilen sonuçlar literatür verileriyle kıyaslandı.
