The Leinbach hip prosthesis performed for hip fractures with impairment of femoral calcar

Between 1994-1996 in orthopaedics clinic of Dokuz Eylül University of Medical Faculty the Leinbach prosthesis had been performed in 27 patients who had unstable intertrochanteric and pertrochanteric fractures with impaired posteromedial support. Mean age was 76.7 (44-95). 18 of patients were female, 9 were male. After the operations, all patients were allowed to sit in their bad at first day and they were mobilized with partial loading at 2nd-3nd days. Mean follow-up was 17 months. 24 of patients postoperatively returned to their preoperative activities. One patient died in our clinic and two patients died in an average of 6 months after they had been discharged, due to respiratory and cardiac problems. Neither dislocation, infection, loosening of stem nor acetabular protrusion occurred during follow-up period.The impairment of femoral calcar integrity according to fracture type should be an effective factor for selection of the type of prosthesis which will be performed. The Leinbach hip prosthesis looks like an appropriate alternative in these patients when the cost of the therapy was also considered.

1994-1996 yılları arasında kliniğımizde posteromedial desteğin bozulduğu instabil intratrokanterik ve pertrokamerik kırığı olan 27 hastaya Leinbach tipi protez uygulanmıştır. Hastaların ortalama yaşı 76.7 (44-95) dir. Olguların 18'i (%66.6) kadın, 9'u (%33.3) erkektir. Hastaların bütünü postoperatif birinci günde yatak içinde oturtulmuş, 2-3. günlerde parsiyel yüklenme ile mobilize olmuşlardır. Hastalar ortalama 7. (5-23) günde kliniğimizden taburcu edilmişlerdir. Ortalama takip süresi 17 aydır. Postoperatif hastaların 24'ü (%88.8) eski yaşamlarına geri dönmüşlerdir. Bir hasta kliniğimizde kaldığı süre boyunca, iki hasta da taburcu olduktan ortalama 6 ay sonra solunumsal ve kardiak problemleri nedeniyle kaybedilmiştir. Takip süresi boyunca hastaların hiçbirinde dislokasyon, enfeksiyon, stemde gevşeme, asetabuler protrüzyonla karşılaşılmamıştır. Kırık tipine göre femoral kalkarın bütünlüğünün bozulması, uygulanacak protez tipinin seçiminde etkili bir faktör olmalıdır. Uygulanacak tedavinin maliyeti de göz önüne alındığında bu tür hastalarda Leinbach kalça protezi uygun bir seçenek olarak görünmekledir.
