The significance of and indications for Chiari's Medial Displacement Osteotomy in the treatment of Acetabular Dysplasia due to Congenital Dislocation or subluxation of the Hip

Öz

Indications for and importance of Chiari's medial displacement osteotomy in the treatment of acetabular dysplasia related to congenital dislocation or subluxation of the hip. Clinical and radiological analyses have been performed preoperatively and at the end of at least one year of follow-up of 24 hips in 20 cases presenting with acetabular dysplasia due to congenital dislocation and subluxation of the hip and were treated with Chiari's medial displacement osteotomy and the findings have been evaluated by Colton's method. The cases in which the youngest was 2 years old and the oldest 8 during operation, were followed up with an average 1, 8 years. The results of Chiari operation are as follows= 12 excellent (%50), 5 good (%20,8), 3 fair (%12,5), 4 poor (%16,7). When performed in strict conformity with its rules, Chiari's medial displacement osteotomy has been observed to be a selective procedure in the treatment of cases presenting with acetabular dysplasia due to congenital dislocation or subluxation of the hip in these older than 4 years of age.

1975 - 1979 yılları arasında İst. Tıp Fak. Ortopedi ve Travmatoloji Kliniğinde DKÇ ve kalça sublüksasyonuna bağlı asetebulum displazisi gösteren ve CHIARI medial deplasman osteotomisi ile tadavi edilen 20 olguya ort 24 kalçanın ameliyat öncesi ve en az bir yıllık takip sonunda klinik ve radyolojik analizleri yapıldı ve bulgular COLTON yöntemi ile değerlendirildi. Ameliyat sırasında en küçüğü 2 en büyüğü 8 yaşında olan olgular, ortalama 1, 8 yıl takip edildi, 24 kalçada uy;gulanan CHIARI ameliyatının sonuçları 12 çok iyi (%50), 5 iyi (%20,8) 3 orta (%12,5) ve 4 kötü (%16,7) şeklindedir. Kaidelerine uyularak yapıldığı zaman CHIARI medial deplasman oeteotomisinin, 4 yaşından sonraki doğuştan kalça çıkığı ya da sublüksasyonuna bağlı asetebular displazi gösteren olguların tedavisinde seçkin bir yönden olduğu görüldü.
