Evaluation of Proliferation Index in Osteosarcoma of Jaws

Aim: Osteosarcoma is the most common primary malignant bone tumour that affects frequently long bones of young people. Approximately 6 % of all ostesarcoma occurs in jaw bones. Jaw osteosarcoma is generally accepted to be a pathologically distinct entity due to older age at presentation, longer median survival, local recurrences and low incidence of metastasis. It has been suggested that better prognosis of jaw osteosarcoma of the jaw bones those in the long bones, may related to the type and the lower stage of the tumour. In this study, proliferation rate of tumour cells in jaw osteosarcoma with comparing long bone osteosarcoma and possible correlation with grading, cellularity and histological type were discussed. Materials and Method: Immunohistochemistry was performed by Ki-67 antibody using avidin biotin complex, in a total of 10 ostesarcoma cases and detection of the cells at proliferative phase was performed. Results: Results of the study displayed the increased Ki-67 expression in long bone osteosarcoma cases when compared to the jaw osteosarcoma. The histological grades of jaw osteosarcomas were lower than long bone osteosarcomas. Conclusion: These results may be related to the grade of the tumours and explain the different biological behaviours. Using proliferation index as a prognostic marker in jaw osteosarcoma could be useful for further researches.

Çene Osteosarkomlarında Proliferasyon İndeksinin Değerlendirilmesi

Amaç: Osteosarkom, genelde uzun kemikleri tutan, en sık görülen primer kemik tümörüdür. Genç bireylerde daha sık görülür. Osteosarkomların % 513'ü çene kemiklerinde ortaya çıkmaktadır. Çene osteosarkomları, daha geç yaşlarda görülmeleri, daha uzun hayatta kalma süreleri, lokal nüks ve geç metastaz gibi özellikleri ile uzun kemik osteosarkomlarından ayrılırlar. Çene osteosarkomlarının, uzun kemik osteosarkomlarından daha iyi bir prognoza sahip olması, tümor tipi ve düşük evreleri ile ilişkili olabilir. Bu çalışmada, çene osteosarkomlarında tümör hücrelerinin proliferasyon (çoğalma) indeksi uzun kemik osteosarkomlarının çoğalma indeksi ile karşılaştırılmış ve tümörün histolojik tipi, sellüleritesi ve evre ile olası ilişkisi tartışılmıştır. Gereç ve Yöntem: Toplam 10 adet osteosarkom vakası Ki-67 antikoru ile avidin biotin kompleks yöntemi ile immünhistokimyasal olarak boyanmış ve proliferatif fazdaki hücreler sayılıp, derecelendirme yapılmıştır. Çene osteosarkomları ile uzun kemik osteosarkomlarının proliferasyon indeksleri birbiri ile kıyaslandığı gibi, klinik bulgular, histolojik tip ve sellülerite ile ilişkisi gözden geçirilmiştir. Bulgular: Çalışmamızda, uzun kemik osteosarkomlarında Ki-67 ekspresyonun çene osteosarkomlarına kıyasla daha fazla olduğu gösterilmiştir. Aynı zamanda çene osteosarkomlarının histolojik evreleri de uzun kemik osteosarkomlarından daha düşük olarak saptanmıştır. Sonuç: Artmış proliferasyon indeksi uzun kemik osteosarkomlarında izlenen yüksek evre ile ilişkili olabilir ve bu iki farklı lokalizasyondaki tümörün sergilediği farklı biyolojik davranışları açıklayabilir. Çene osteosarkomlarında prognostik belirteç olarak proliferasyon ideksinin yerinin ileride yapılacak araştırmalar için de faydalı olacağı düşünülmektedir.
