TEMPORO-MANDİBULAR EKLEM PROBLEMLERİNİ DEĞERLENDİRMEDE DİŞ HEKİMLERİ ARASINDA HERHANGİ BİR FARK VAR MI?

AMAÇ: Son yıllarda, temporomandibular eklem düzensizlikleri (TMD) ile ilişkili oro-fasiyal ağrı şikayeti olan hasta sayısında belirgin bir artış olmuştur. Bu çalışmanın amacı, Atatürk Üniversitesi Diş Hekimliği Fakültesi araştırma görevlilerini, TMD uzmanlarına ve genel diş hekimi pratisyenlerine (GDP’ler) ayırmak ve bilgi, tutum ve pratik yeterliliklerini değerlendirmektir. GEREÇ VE YÖNTEM: Ankete Atatürk Üniversitesi Diş Hekimliği Fakültesi'nden toplam 80 araştırma görevlisi (33 TMD uzmanı ve 47 GDP’ler) katılmıştır. TMD'lerle ilgili 4 bölüm ve 22 sorudan oluşan bir anket, ilgili standart ders kitaplarından oluşturulmuştur. Anketteki bu bölümler kimlik, bilgi, tutum ve uygulama bölümleridir. Her iki grubun bilgi ve tutum puanları değerlendirildi ve karşılaştırıldı. Gruplar ayrıca uygulama bölümünde terapötik yöntemler ve kendilerini güncelleme yöntemleri hakkında değerlendirildi. İstatistiksel analiz Chi kare testi kullanılarak yapıldı. BULGULAR: TMD uzmanları ile GDP’ler arasındaki bilgi bölümü puanlarında anlamlı bir fark bulunmuştur (p <0.005) Her iki grupta da düşük bilgi skoru tespit edilmedi. Ayrıca, GDP'lerde yüksek bilgi skoru ve TMD uzmanlarında ise orta bilgi skoru tespit edilmedi. Ayrıca her iki grup arasında tutum bölümünde anlamlı bir fark bulunmuştur (p <0.001) İki grup, oklüzal çatışmaların giderilmesi, ortodontik tedavinin başlatılması ve tanı amaçlı radyografi konusunda ciddi bir anlaşmazlığa düşmüştür. Ayrıca, kullanılan terapötik yöntemler ve güncelleme kaynakları konusunda her iki grup arasında da farklılıklar bulunmuştur. GDP'lerin yaklaşık %72'si yetersiz sayıda TMD uzmanı ile ilgili endişelerini dile getirdi. SONUÇ: TMD'lerin teşhis ve tedavi seçeneklerini belirlemek bazen diş hekimleri için büyük bir zorluktur ve bunların bilgi, tutum ve pratik yetenekleri ile orantılı olduğu düşünülmektedir. Lisans ve lisansüstü eğitim programlarının revizyonu, bilgi ve kongre eğitimlerinin güncellenmesi önemlidir. Anahtar Kelimeler: temporo-mandibular eklem problemi, Diş hekimi, Temporomandibular IS THERE ANY DIFFERENCE BETWEEN DENTISTS IN EVALUATING THE TEMPORO-MANDIBULAR JOINT PROBLEMS? ABSTRACT OBJECTIVES: In recent years, there has been a marked increase in the number of patients who have complained of oro-facial pain associated with temporomandibular disorder (TMDs). The aim of this study is to separate Atatürk University Faculty of Dentistry’s research assistants to TMD experts and general dental practitioners (GDPs) and evaluate their knowledge, attitude and practical competence. MATERIALS AND METHODS: A total of 80 research assistants (33 TMD experts and 47 GDPs) from Atatürk University Faculty of Dentistry participated in this survey. A questionnaire consisting of 4 sections and 22 questions with regard to TMDs was designed from relevant standard textbooks. These sections of the questionnaire are identity, knowledge, attitude and practical sections. The knowledge and attitude scores of both groups were assessed and compared. The groups were also evaluated in the practice section about the therapeutic modalities and the methods of updating themselves. Statistical analysis was performed using Chi square test. RESULTS: A significant difference was found in the knowledge section scores between TMD experts and GDPs. (p<0.005) There was no low score in both groups. There was also no high score in GDPs and a fair score in TMD experts. In addition, a more significant difference was found in attitude section between both groups. (p<0.001) The two groups have fallen into serious disagreement about the removal of occlusal interferences, initiation of orthodontic treatment and radiography for diagnosis. Moreover, there was also found differences between both groups about the therapeutic modalities used and update sources. Around 72% of GDPs expressed concern on inadequate number of TMD experts. CONCLUSION: Determining the diagnostic and therapeutic options of TMDs is sometimes a major challenge for dentists and is thought to be proportional to the knowledge, attitude and practical ability of them. Revision of under-graduate and post-graduate education programs, updating of knowledge and congress trainings are important.
