MICROBIOLOGIC EVALUATION OF ACUTE EXACERBATION DURING ROOT CANAL TREATMENT

Öz

Many studies reported the relationship between the pressence of microorganisms in pulp and formation of periapical diseases. Microbial factor may be combined with iatrogenic factors, however microbial factor can cause pain alone in spite of proper application of root canal treatment procedures. Certain bacterial species have been linked with clinical features of periradicular diseases. Bacteria and their products being pushed into periradicular tissues may lead acute inflammation. Unfinished chemomechanical preparation may damage the balance by elimination of inhibitor species and causing proliferation of previously inhibited species. Root canal treatment procedures are inevitable to cause differences in root cana microbiota. Theorically, infected cases create risks for flare-up therefore clinician may apply some procedures to prevent or minimally decrease insidence of flare-ups

Öz

Pulada mikroorganizma olmasının periapikal hastalık oluşumuyla doğrudan ilişkisi olduğu birçok araştırmada gösterilmiştir. Mikrobiyolojik faktörler iatrojenik faktörler ile kombine edilebilir ancak mikro- biyal içerik; kök kanal prosedürleri eksiksiz bir şekilde uygulansa dahi ağrıya neden olabilir. Belirli bakteri türleri varlığının, periradiküler hastalığın klinik özellikle- riyle ilişkili olduğu daha önce ortaya koyulmuştur. Bakterilerin ve onların ürünlerinin periradiküler doku- lara itilmesi, akut enflamatuar cevap oluşturabilir. Tamamlanmamış kemomekanik preperasyon bazı inhibe edici türleri eleyerek daha önceden inhibe olmuş türlerin tekrar çoğalmasını sağlayarak, mikro- biyal komitedeki dengeyi bozabilir. Endodontik prosedürlerin kök kanal içerisinde bulunan ekolojik dengede değişiklik meydana getirmesi kaçınılmazdır. Teorik olarak devital vakalar seanslar arası flare-up için risk oluşturur, hekim bu durumu önlemek, en azından oluşma sıkılığı azaltmak için bazı önlemler almalıdır.
