Evde Sağlık Hizmeti Alan Bireylerde Serum Albümin ve Üriner İnkontinans İlişkisi; Tek Merkez Deneyimi

Öz

Amaç: Bu çalışma, evde sağlık hizmeti (ESH) alan yaşlı bireylerde serum albümin düzeyleri ile üriner inkontinans (Üİ) arasındaki ilişkiyi değerlendirmeyi amaçlamıştır.

Yöntemler: Giresun Eğitim ve Araştırma Hastanesi EBH biriminde 15 Nisan-15 Mayıs 2024 tarihleri arasında takip edilen, Üİ tanılı 49 hasta ile yaş ve cinsiyet uyumlu 53 Üİ olmayan kontrol hastasının verileri retrospektif olarak analiz edildi. Demografik ve laboratuvar verileri elektronik sağlık kayıtlarından elde edildi. Sürekli değişkenler ortalama ± standart sapma (SS), kategorik değişkenler frekans ve yüzde olarak ifade edildi. Grup karşılaştırmalarında kategorik değişkenler için Ki-kare veya Fisher kesin testi, sürekli değişkenler için bağımsız t-testi kullanıldı. p<0.05 istatistiksel olarak anlamlı kabul edildi.

Bulgular: Kohortun yaş ortalaması 80.6 ± 13.0 yıl olup %68.6'sı kadındı. Üİ ve kontrol grupları arasında serum albümin düzeylerinde anlamlı fark gözlenmedi (37.1 ± 6.8 g/L vs 37.6 ± 5.1 g/L; p = 0.687). Ancak Üİ grubunda serum T4 düzeyleri anlamlı derecede düşüktü (1.2 ± 0.2 μg/dL vs 1.4 ± 0.4 μg/dL; p = 0.001). Diğer laboratuvar parametreleri gruplar arasında farklılık göstermedi.

Sonuç: Bu yaşlı evde bakım popülasyonunda serum albümin düzeyleri ile Üİ arasında anlamlı bir ilişki saptanmamıştır. Ancak T4 düzeylerindeki belirgin fark, tiroid disfonksiyonunun Üİ patofizyolojisinde rolü olabileceğini düşündürmektedir. Bu bulguların doğrulanması için daha geniş örneklemli prospektif çalışmalara ihtiyaç vardır.

The Relationship Between Serum Albumin and Urinary Incontinence in Home Healthcare Patients: A Single-Center Experience

Öz

Objective: This study aimed to evaluate the association between serum albumin levels and urinary incontinence (UI) in elderly individuals receiving home care services (HCS).
Methods: A retrospective analysis was conducted using data from 49 patients diagnosed with UI and 53 age- and sex-matched controls without UI, who received care at Giresun Training and Research Hospital’s HCS between April 15 and May 15, 2024. Demographic and laboratory data were extracted from electronic health records. Continuous variables were expressed as mean ± standard deviation (SD), and categorical variables as frequencies and percentages. Group comparisons were performed using the Chi-square or Fisher’s exact test for categorical variables, and independent t-tests for continuous variables. A p-value of <0.05 was considered statistically significant.
Results: The mean age of the cohort was 80.6 ± 13.0 years, and 68.6% were female. No significant difference was observed in serum albumin levels between the UI and control groups (37.1 ± 6.8 g/L vs 37.6 ± 5.1 g/L; p = 0.687). However, serum T4 levels were significantly lower in the UI group (1.2 ± 0.2 μg/dL vs 1.4 ± 0.4 μg/dL; p = 0.001). Other laboratory parameters did not differ significantly between groups.
Conclusion: Serum albumin levels were not significantly associated with UI in this elderly home care population. However, the significant difference in T4 levels suggests a potential role of thyroid dysfunction in UI pathophysiology. Prospective studies with larger sample sizes are warranted to validate these findings.
