Psikotrop ilaçların metabolik yan etkileri ve yönetimi

Öz

Psikiyatrik bozukluğu olan bireylerde mortalite ve morbidite oranlarının genel popülasyona kıyasla daha yüksek seyretmesinde, hastalığın doğrudan etkilerinin yanı sıra tedavi sürecinde ortaya çıkabilen metabolik bozulmalar önemli bir rol oynamaktadır. Psikotrop ilaçların uzun süreli kullanımı, özellikle bazı ajanlarda daha belirgin olmak üzere, metabolik açıdan klinik olarak anlamlı yan etkilere yol açabilmektedir. Bu yan etkiler kilo artışı ve santral yağlanmadan dislipidemi, insülin direnci, diyabet ve hipertansiyona kadar uzanan geniş bir spektrumu kapsamakta; söz konusu bozuklukların bir araya gelmesiyle metabolik sendrom gelişebilmektedir. Uzamış tedavi süreçlerinde bu metabolik değişiklikler ciddi sağlık sorunlarına zemin hazırlayabilmektedir. Bu nedenle metabolik yan etkilerin düzenli aralıklarla taranması, yakından izlenmesi ve erken dönemde tanınarak uygun şekilde yönetilmesi tedavinin ayrılmaz bir parçasıdır. Psikotrop ilaçlarla tedavi edilen bireylerde, özellikle risk grubunda yer alan hastalarda, ilaç seçiminin dikkatle yapılması, yaşam tarzına yönelik müdahalelerin planlanması ve gerekli durumlarda ek farmakolojik yaklaşımların devreye sokulması önerilmektedir. Metformin, topiramat ve glukagon benzeri peptid-1 (GLP-1) agonistleri gibi ajanların, psikotrop ilaçlara bağlı kilo artışı ve metabolik bozulmaların kontrol altına alınmasında yararlı olabileceğine dair bulgular mevcuttur. Klinik karar verme sürecinde bireysel risk profili ve tedaviye verilen yanıt göz önünde bulundurulmalı; bütüncül bir yaklaşım benimsenerek hem psikiyatrik belirtilerin hem de metabolik sonuçların iyileştirilmesi hedeflenmelidir. Bu derlemede, psikotrop ilaç kullanımına eşlik edebilen metabolik yan etkiler ile bu yan etkilerin farmakolojik ve ilaç dışı yönetim stratejileri ele alınmıştır.

Metabolic side effects and management of psychotropic drugs

In individuals with psychiatric disorders, the higher rates of mortality and morbidity compared with the general population are influenced not only by the direct effects of the illness itself but also by metabolic disturbances that may emerge during the course of treatment. The long-term use of psychotropic medications, particularly certain agents, can lead to clinically significant metabolic adverse effects. These effects encompass a broad spectrum, ranging from weight gain and central adiposity to dyslipidemia, insulin resistance, diabetes mellitus, and hypertension; the coexistence of these abnormalities may result in the development of metabolic syndrome. Over prolonged treatment periods, such metabolic changes can predispose individuals to serious health complications. Therefore, regular screening, close monitoring, early identification, and appropriate management of metabolic adverse effects constitute an integral part of treatment. In individuals receiving psychotropic medications especially those at increased risk careful selection of pharmacological agents, implementation of lifestyle interventions, and, when necessary, the introduction of adjunctive pharmacological strategies are recommended. Evidence suggests that agents such as metformin, topiramate, and glucagon-like peptide-1 (GLP-1) receptor agonists may be beneficial in controlling psychotropic-induced weight gain and metabolic disturbances. In the clinical decision-making process, individual risk profiles and treatment responses should be taken into account, and a holistic approach should be adopted with the aim of improving both psychiatric symptoms and metabolic outcomes. This review addresses the metabolic adverse effects associated with psychotropic medication use, as well as the pharmacological and non-pharmacological strategies for their management.
