May Hypothermia Associated with Taking Shower?

Hypothermia is a life threatening yet preventable condition where the normal body temperature drops below 35°C. Hypothermia can be due to accidental or intentional (like in the cases of coronary artery by-pass surgeries) and can be classified as primary (exposure to cold temperatures) or secondary (hypothermia due to another health disorder) hypothermia. Although in most studies fatal hypotherima is associated with winter conditions, it may not always take place in cold climate situations. Surprisingly, there are numerous cases reported from areas of temperate climates or inexposure to external conditions. Our case is interesting because the patient took a shower in october on a day with average normal temperature that led to hypotherima followed by death. Due to this case, we decided to review hypothermia etiology, differential diagnosis and emergency room approaches.

Hipotermi vücut merkez ısısının 35C°’nin altında olmasıdır ve hayatı tehdit eden nadir ve önlenebilir bir durumdur. Hipotermi kazara ya da koroner arter bypas ameliyatlarında olduğu gibi maksatlı (intentional) ve primer (soğuğa maruziyetteki gibi) ya da sekonder (başka bir hastalığa bağlı olarak) hipotermi şeklinde sınıflandırılabilir. Çalışmalarda ölümcül hipotermilerin çoğunluğunun kış aylarında olduğu söylense de her zaman soğuk iklimlerle bağlantı olmayabilir. Şaşırtıcı şekilde ılımlı iklime sahip yerlerde veya dış ortama maruziyeti olmayan durumlarda da çok sayıda vaka meydana gelmektedir. Olgumuz ekim ayında ortalama sıcaklıkta bir günde duş aldıktan sonra hipotermiye girmesi ve ölümle sonuçlanması nedeniyle ilginçtir. Hipotemiye sebep olan veya yatkınlık oluşturan nedenlerin ayrımında yaşadığımız zorlukları, etyolojisini ve acil servis yaklaşımını bu olgu vesilesiyle gözden geçirmeyi amaçladık.
