COURSE OF GLOMERULONEPHRITIS; RELATIONSHIP WITH QUALITY OF LIFE AND DEPRESSION

Öz

Materials and Methods: Twenty-nine glomerulonephritis diagnosed patients
were included in the study by referring to Nephrology Clinic. Short Form 36 (SF36), Hospital Anxiety and Depression Scale (HAD) were administered at the time
of the disease diagnosis, at 3rd and 6th months of treatment and follow-up. It was
evaluated by comparing with the laboratory parameters examined simultaneously.
Results: The mean age was 44±17 years. There was a statistically significant
difference between depression and QoL and proteinuria at baseline, third and
6th months. When the patients were evaluated in terms of remission at the 6th
month, the overall health, anxiety and pain parameters of 22 patients who had
partial and complete remission were found to be significantly different from
those without remission. Hemoglobin level was the most effective parameter in
the initial depression and quality of life scores, and the decrease in the amount of
proteinuria was the most effective factor in the significant improvement in
depression scores between 3 and 6 months.
Conclusion: In the presence of proteinuria in adult glomerulopathy patients
without renal dysfunction, our study emphasized the importance of proteinuria
and its relation with low quality of life, high depression and anxiety scores once
more.

Glomerülonefritin seyri; yaşam kalitesi ve depresyon ile ilişkisi

Öz

Amaç: Glomerulonefritler, kronik böbrek hastalığının önemli nedenlerinden biri
olduğundan hastalarda ölüm korkusu, anksiyete ve depresyon görülme
sıklığının artması beklenen bir durumdur. Hastalık şiddetinin en önemli
prognostik bulgusu olan proteinüri ile yaşam kalitesi ve depresyon arasında
ilişki olup olmadığını ortaya koymak için bu çalışma planlanmıştır.
Gereç ve Yöntem: Nefroloji Kliniğinde glomerulonefrit tanısı alan 29 hasta
çalışmaya dâhil edildi. Hastalara tanı anında, tedavi ve izlemin 3. ve 6. ayında
olmak üzere Kısa Form 36 (SF-36) ve Hastane Anksiyete ve Depresyon Ölçeği
(HAD) uygulandı. Eş zamanlı bakılan laboratuvar parametreleri ile ilişkileri
değerlendirildi.
Bulgular: Hastaların yaş ortalaması 44±17 yıl idi. Başlangıç, 3. ay ve 6. aylarda
depresyon ve yaşam kalitesi ile proteinüri arasında istatistiksel olarak anlamlı
farklılık izlendi. Hastalar 6. ayda remisyon açısından değerlendirildiğinde kısmi
ve tam remisyona giren 22 hastanın genel sağlık, anksiyete ve ağrı
parametrelerinin remisyona girmeyenlere göre anlamlı farklı olduğu görüldü.
Başlangıçtaki depresyon ve yaşam kalitesi puanlarında en etkin parametrenin
hemoglobin düzeyi, 3. ve 6. ay arasındaki depresyon puanlarındaki anlamlı
iyileşmede en etkin faktörün de proteinüri miktarındaki azalma olduğu görüldü.
Sonuç: Erişkin glomerulopati hastalarında proteinüri varlığında, böbrek
fonksiyon bozukluğundan bağımsız olarak, düşük yaşam kalitesi, yüksek
depresyon ve anksiyete skorlarının saptandığı çalışmamız proteinürinin önemini
bir kez daha vurgulamıştır.
