COULD VITAMIN D DEFICIENCY BE A PREDISPOSING FACTOR FOR THE DEVELOPMENT OF UTERINE FIBROIDS?

Öz

Objective: The objective of this study is to determine the relationship between low serum 25-hydoxyvitamin D concentrations and uterine fibroids in women.
Materials and Methods: Medical records of women, who visited the outpatient clinic of the department of obstetrics and gynecology between January 2017 and December 2019, were examined. Records of cases, who met the inclusion criteria, were analysed. Serum 25-hydroxyvitamin D concentrations of cases with uterine fibroids and healthy controls were compared. Measurements of 20 ng/mL and above were considered normal, while concetrations lower than 20 ng/mL were considered as vitamin D deficiency.
Results: Serum 25-hydroxyvitamin D concentrations of 111 cases of uterine fibroids and 111 controls were compared. Groups were similar regarding the sociodemographic characteristics, age, body mass index and parity. The mean age was 41.9±7.00 years for both groups. The mean serum 25-hydroxyvitamin D concentration was 17.80±10.90 ng/mL in the leiomyoma group and 20.86±10.20 in the control group. The leiomyoma group was divided into two groups: small-sized leiomyomas (<150 cm3, 67 cases) and large-sized leiomyomas (≥150 cm3,44 cases). Mean serum 25-hydroxyvitamin D levels were lower in cases with large-sized leiomyomas (16.73±10.11 ng/mL), while higher in patients with small-sized leiomyomas (18.81±11.36ng/mL). The difference was not statistically significant.
Conclusion: Serum 25-hydroxyvitamin D levels of patients with uterine leiomyomas were lower than control group in our study. Although this difference is not statistically significant, it is consistent with the findings of many previous studies. Further studies with more cases are needed to conclude that vitamin D deficiency is a predisposing factor for uterine leiomyoma development.

D vitamini eksikliği uterin fibroid gelişimi için predispozan bir faktör olabilir mi?

Öz

Amaç: Çalışmadaki amacımız, kadınlarda düşük serum 25–hidroksivitamin D düzeylerinin, uterin leiomyom görülme sıklığı ile ilişkisinin belirlenmesidir.
Gereç ve Yöntem: Çalışma kapsamında, Ocak 2017 – Aralık 2019 tarihleri arasında, kadın hastalıkları ve doğum polikliniğine çeşitli nedenlerle başvuran kadınların medikal kayıtları incelendi. Çalışmaya dâhil olma kriterlerini karşılayan, uterin leiomyom saptanan olgular ile sağlıklı kontrol grubu olguların serum 25 – hidroksivitamin D düzeyleri karşılaştırıldı. 20 ng/mL ve üzeri ölçümler normal düzey kabul edilirken, 20 ng/mL altı düzeyler subklinik vitamin D eksikliği olarak değerlendirildi.
Bulgular: Çalışma kriterlerini karşılayan 111 uterin leiomyomlu olgu ile benzer sosyodemografik özelliklere sahip 111 olgunun serum 25–hidroksivitamin D düzeyleri karşılaştırıldı. Gruplar yaş, vücut kitle indeksi, abortus ve parite açısından benzerdi. Her iki grubun ortalama yaşı 41,90±7,00 olarak saptandı. Leiomyom grubunda ortalama serum 25 - hidroksivitamin D düzeyleri 17,80±10,90 ng/mL, kontrol grubunda ise 20,86±10,20 ng/mL olarak ölçüldü. Leiomyom boyutları dikkate alınarak olgular küçük boyutlu leiomyomlar (<150 cm3, 67 olgu) ve büyük boyutlu leiomyomlar (≥150 cm3, 44 olgu) olarak iki gruba ayrıldı. Büyük boyutlu leiomyomlarda ortalama serum 25-hidroksivitamin D düzeyleri (16,73±10,11ng/mL) daha düşük saptanırken, küçük boyutlu leiomyomlarda(18,81±11,36 ng/mL) daha yüksek saptandı. Fark istatistiksel olarak anlamlı değildi.
Sonuç: Çalışma popülasyonumuzda, leiomyomlu olguların serum 25 – hidroksivitamin D düzeylerinin, kontrol grubu sağlıklı kadınlara göre, daha düşük seviyelerde olduğunu saptadık. Bu fark istatistiksel olarak önemli olmasa da literatürde yer alan birçok çalışmanın bulgularıyla uyumludur. Düşük serum 25 – hidroksivitamin D düzeylerinin uterin leiomyom gelişimine predispozan bir faktör olduğunu söylemek için daha fazla olguya sahip yeni çalışmalar gereklidir.
