Türkiye’deki Dil ve Konuşma Terapistlerinin Çevrimiçi Terapilere Yönelik Görüş ve Deneyimlerinin İncelenmesi

Öz

Amaç: COVID-19 pandemisiyle hız kazanan tele-terapi, Dil ve Konuşma Terapisi alanında erişilebilirlik ve esneklik sağlasa da teknik, etik ve iletişimsel sınırlılıklar içermektedir. Bu çalışma Türkiye’deki dil ve konuşma terapistlerinin (DKT) çevrimiçi terapiye yönelik tutumlarını, deneyimlerini ve uygulamalarını incelemeyi amaçlamaktadır.

Yöntem: Araştırma nicel betimsel desenle yürütülmüştür. Veriler alanyazın temelli ve uzman görüşüyle geçerliliği sağlanmış 29 maddelik anket aracılığıyla toplanmıştır. Ankete 140 DKT (119 kadın, 21 erkek; Ort. yaş = 25.62, SS = 3.85) katılmıştır. Veriler frekans ve yüzde dağılımlarıyla özetlenmiş; çevrimiçi terapi deneyimi ile terapi tercihi arasındaki ilişkiyi incelemek için ki-kare (χ²) testi uygulanmıştır. Anlamlı farklılıkların hangi hücrelerden kaynaklandığını belirlemek amacıyla post-hoc analizler yapılmış ve standartlaştırılmış artıklar (z-skorları) raporlanmıştır. Anlamlılık düzeyi p < .05 olarak kabul edilmiştir.

Bulgular: Katılımcılar çevrimiçi terapiye yönelik farklı tutumlar sergilemiştir. Esneklik (%82.8) ve maliyet (%63.6) öne çıkan avantajlardır; ancak yalnızca %33’ü yasal düzenlemeler konusunda “her zaman” bilgi sahibi olduğunu belirtmiştir. Deneyim süresi ile terapi tercihi arasında anlamlı ilişki bulunmuştur, χ²(6, N = 140) = 37.72, p < .05). Üç–altı yıl deneyime sahip DKT’ler çevrimiçi terapiyi daha çok tercih etmiştir (z = 3.86, p <.001). Zoom en yaygın kullanılan platform olmuştur (%75); ancak yalnızca %14’ü sertifikasyona sahiptir. Vaka türlerinde en sık konuşma sesi bozuklukları (%38) ve akıcılık bozuklukları (%28) görülmüş, ergenler en büyük yaş grubunu oluşturmuştur (%39). Katılımcıların yarısından fazlası çevrimiçi terapilerde ses (%52,9) ve görüntü (%52.1) kalitesini düşük bulmuştur. Genel tercih açısından, %51’i yüz yüze terapiyi önde tutarken yalnızca %4’ü çevrimiçi terapiyi “genellikle” tercih etmiştir.

Sonuç: Çevrimiçi terapi Türkiye’de önemli fırsatlar sunmakla birlikte, teknik sorunlar, terapötik bağın zayıflaması ve yasal/etik belirsizlikler nedeniyle sınırlılıklar barındırmaktadır. Mesleki deneyim, olumlu tutumlarda belirleyici bir faktördür. Çevrimiçi terapi yüz yüze terapinin yerine geçen değil, tamamlayıcı bir yöntem olarak görülmelidir. Altyapının güçlendirilmesi, standart sertifikasyon programlarının oluşturulması ve yasal çerçevenin netleştirilmesi gelecekte öncelikli hedeflerdir.

Araştırmacı, etik kurallarına uygunluğu sağlamak amacıyla hazırladığı etik kurul başvuru belgeleriyle Ankara Medipol Üniversitesi Etik Kurul Başkanlığı’na başvurmuş ve etik kurul onayı almıştır (08.01.2024 tarihli toplantı, 3 numaralı karar, Protokol No: E-81477236-604.01.01-134).

An Investigation of Speech and Language Therapists’ Opinions and Experiences Regarding Online Therapy in Türkiye

Öz

Purpose: The aim of this study is to examine the opinions and experiences of speech and language therapists in Turkey regarding online therapy, and to identify the platforms they use and the challenges they encounter.
Method: This study was designed using a descriptive research approach and involved the collection of quantitative data through an online survey. A total of 140 speech and language therapists from across Turkey participated in the study. Participants were asked to provide information on their opinions about online therapy, preferred platforms, length of experience, and challenges faced. Quantitative data collected through a questionnaire developed by the researcher were analyzed using frequency tables and the Chi-square test. The statistical analyses were conducted using SPSS version 29.0.2.0, and a significance level of p<0.05 was used for the interpretation of the results.
Results: The majority of speech and language therapists reported preferring Zoom for online therapy. The most frequently addressed speech and language issues were speech sound disorders and fluency disorders, primarily in individuals aged 13 to 17. Most participants did not have any specific training or certification in online therapy. Poor audio and video quality were commonly reported issues. Technical problems, lack of non-verbal cues, and difficulties in using technology were prominent challenges for many therapists. However, most participants agreed that online therapy provides flexibility in scheduling and cost advantages.
Conclusion: The findings of this study offer valuable insights into the tendencies and experiences of speech and language therapists in Turkey regarding online therapy. The results highlight how online therapy is being effectively implemented in clinical practice and point to potential areas for further development in this field.
