Pulpa Taşları ve Sistemik Hastalıklar: Derleme

Öz

Pulpanın sert doku reaksiyonları; kalsifikasyonlar, artan dentin oluşumları ve rezorpsiyonlardır. Pulpada kalsifiye oluşumlara sıklıkla rastlanmaktadır. Pulpa taşı kökün koronal veya apikal kök kısmında görülebilen, pulpa odasında olduğunda kanala girişi ve kanalların lokalizasyonunu zorlaştırabilen, kök pulpasında olduğunda ise kanal tedavisinin fizyolojik foramene kadar yapılmasını engelleyebilen kalsifik oluşumlardır. Yaşlanma, çürük, dental işlemler, periodontal hastalıklar, pulpada epitel artıkları varlığı, ortodontik hareketler, idiyopatik veya genetik faktörlerle oluşabildiği düşünülmektedir ancak kesin nedeni tam olarak bilinmemektedir. Pulpa kalsifikasyonlarının dental nevraljiye neden olduğu öne sürülmektedir ve gelişimde bir bozukluk veya pulpa dokusu içinde lokal bir patoloji olarak değerlendirilmelidir. Patolojik yapılarına rağmen vücuttaki tüm ektopik kalsifikasyonlar içinde en küçük ve zararsız olanlarıdır. Literatürde pulpa taşının böbrek taşı, karotis arter kalsifikasyonu, safra kesesi taşı gibi diğer kalsifik oluşumlara neden olan çeşitli sistemik hastalıklarla ilişkisini değerlendiren çalışmalar mevcuttur. Bu derlemede pulpa taşı tanımı ve türleri, oluşum mekanizmaları, prevalansı, klinik önemi ve pulpa taşı varlığında diş hekiminin gerekli yaklaşımı, dental anomali ve sistemik hastalıklarla ilişkileri hakkında güncel literatür taraması yapılmış ve bu doğrultuda bilgi edinilmiştir.

Öz

Hard tissue reactions of the pulp; calcifications, increased dentin formations and resorption. Calcified formations are frequently encountered in the pulp. Pulp stones are calcific formations that can be seen in the coronal or apical root part of the root, when it is in the pulp chamber, it can make it difficult to enter the canal and localization of the canals, and when it is in the root pulp, it can prevent canal treatment up to the physiological foramen. It is thought to be caused by aging, caries, dental procedures, periodontal diseases, the presence of epithelial residues in the pulp, orthodontic movements, idiopathic or genetic factors, but the exact cause is not known. It is suggested that pulp calcifications cause dental neuralgia and should be considered as a developmental disorder or a local pathology within the pulp tissue. Despite their pathological nature, they are the smallest and harmless of all ectopic calcifications in the body. In the literature, there are studies evaluating the relationship of pulp stone with various systemic diseases that cause other calcific formations such as kidney stones, carotid artery calcification, and gallstones. In this review, the current literature review has been made on the definition and types of pulp stone, its formation mechanisms, prevalence, clinical importance, the necessary approach of the dentist in the presence of pulp stone, its relationship with dental anomalies and systemic diseases, and information has been obtained accordingly.
