Radiological Findings of Spontaneous Urinomas

Abstract

Objective: In this article, the radiologic findings in the processes of diagnosis and treatment of spontaneous urinomas, which represent an uncommon urinary system pathology, were retrospectively evaluated and explicated with published literature together. Materials and Methods: The medical records were searched for all cases of spontaneous urinoma diagnosed between 2000-2007. When records were examined, iatrogenic and post traumatic urinomas were excluded from the study. Finally, four cases were identified as spontaneous urinomas, and all radiologic and urologic features of these cases were evaluated. Results: All cases were male patients with ages between 35-78 (mean:50.8). The most common complain was flank pain (%100). The most common physical examination finding was costovertebral angle tenderness (%75). The initial modality utilized was utrasonographic imaging. Because urinoma formation was suspected in all patients but one with ultrasonography, all patients underwent computed tomographic imaging to make the diagnosis of urinoma. Urinomas in three cases were spontaneously resorbed in follow-up. Conclusion: This study indicated that ultrasonograpghy is the initial method for detecting fluid collection in retroperitoneal space, contrast-enhanced computed tomography does have significant role in verifying the spontaneous urinoma diagnosis and in demonstrating diagnostic details and uncomplicated urinomas may self-resolve without any intervention.

Spontan Ürinomların Radyolojik Bulguları

Öz

Amaç: Bu makalede, nadir bir üriner sistem patolojisi olan spontan ürinomların tanı ve tedavi aşamalarındaki radyolojik bulgular retrospektif olarak incelenmiş ve mevcut literatür eşliğinde irdelenmiştir. Gereç ve Yöntemler: 2000-2007 yılları arasında spontan ürinom tanısı alan tüm olguların kayıtlarına ulaşıldı. Yapılan taramada iyatrojenik veya travma sonucu oluşan ürinom olguları çalışmaya dahil edilmedi. Spontan ürinom tanısı alan 4 olgu tespit edildi ve olguların tüm ürolojik ve radyolojik özellikleri değerlendirmeye tabi tutuldu. Bulgular: Hastaların tamamı erkekti ve 35-78 yaş aralığındaydı (ortalama yaş: 50.8). Olgularda en sık rastlanılan yakınma yan ağrısıydı (%100). En sık olarak saptanan fizik muayene bulgusu tutulan tarafta kostovertebral açı hassasiyetinin pozitif olmasıydı (%75). Ultrasonografik inceleme, tüm olgularda yapılan ilk radyolojik değerlendirmeydi. Biri hariç tüm olgularda, ultrasonografide ürinom formasyonundan şüphelenilmesi üzerine BT inceleme yapılmış ve ürinom tanısına ulaşılmıştı. Takipte, olguların üçünde (%75) ürinomların spontan olarak rezorbe olduğu tespit edildi. Sonuç: Bu çalışma, ultrasonografinin, retroperitoneal alandaki sıvı koleksiyonunu saptamada ilk başvurulacak yöntem olduğunu, kontrastlı bilgisayarlı tomografinin, spontan ürinomun tanısını doğrulamada ve tanısal ayrıntıları göstermede önemli rol oynadığını, komplike olmayan spontan ürinomların herhangi bir girişim gerektirmeksizin kendiliğinden rezorbe olabileceğine işaret etmektedir.
