Abducens Nevre Palsy without Diplopia in a Multiple Sclerosis Patient

Abstract

Multiple sclerosis (MS), is a chronic demyelinating relapsing-remitting disease of central nervous system. In the etiopatogenesis, autoimmunity to myelin plays role. In MS, ocular motor sings are not rare. Internucleer ophthalmoplegia is the most common horizontal gaze disorder in MS patients however sixth nerve palsy is not rare. In acquired cranial ocular nerve palsies, diplopia is an important symptom. However in our forty one-year-old MS patient, with sixth nerve palsy, diplopia symptom was not present. The reason is central scotoma in the left eye. We should keep in mind that during the ocular movement examination of MS patients, diplopia may not be reported because of optic neuritis squeal. ©2008, Firat University, Medical Faculty

Multipl Sklerozlu Olguda Çift Görme Olmadan Altıncı Sinir Tutulumu

Öz

Multipl Skleroz (MS), santral sinir sistemini tutan, alevlenmeler ve sönmelerle seyreden kronik demiyelizan bir hastalıktır. Etiyopatogenezde myeline karşı immün cevap rol oynar. MS’ da motor oküler belirtiler nadir değildir. İnternükleer oftalmopleji, multipl skleroz hastalarında en sık gözlenen horizontal bakış bozukluğu olmasına karşın altıncı sinir paraliziside nadir değildir. Göz hareketinden sorumlu sinirlerin edinsel paralizlerinde en önemli belirti çift görmedir. Ancak sunduğumuz altıncı sinir paralizili kırk bir yaşındaki MS hastasında, çift görme şikayeti yoktu. Çift görme belirtisinin olmaması sol göz görme alanındaki santral skotom ile açıklanabildi. MS hastalarının göz hareket muayeneleri yapılırken çift görme belirtisinin, geçirilmiş optik nörit sekeli sebebiyle tespit edilemeyeceği akılda tutulmalıdır.©2008, Fırat Üniversitesi, Tıp Fakültesi
