POSTSPINAL INTRACRANIAL SUBDURAL HEMATOMA AND CEREBRAL SINUS THROMBOSIS: REPORT OF A CASE

Cerebral venous thrombosis is a rare but a serious complication of spinal anesthesia. It usually occurs in the presence of predisposing factors such as pregnancy, puerperium, use of oral contraceptive drugs, malignancies, thrombocytopenia and the most frequent symptom is headache. Twenty-two years of age, pregnant woman had a complaint of headache after spinal anesthesia for caesarean section and diagnosed as post-dural puncture headache, the treatment was begun. After detecting responsiveness to treatment, radiological imaging procedures were performed and subdural hematoma and transverse sinus thrombosis were detected. In this case report, it was concluded under current literatures that the subdural hematoma and transverse sinus thrombosis should be kept in mind during the diagnosis of post-dural puncture headache.

POSTSPİNAL İNTRAKRANİYAL SUBDURAL HEMATOM VE SEREBRAL SİNUS TROMBOZU : OLGU SUNUMU

Öz

Serebral ven trombozu, spinal anestezinin nadir ancak ciddi bir komplikasyonudur. Çoğunlukla gebelik, lohusalık, oral kontraseptif kullanımı, maligniteler ve trombositopeni gibi predispozan faktörlerin varlığında ortaya çıkmaktadır ve en sık semptom başağrısıdır. Yirmiiki yaşında gebe hastanın, spinal anestezi altında sezeryan operasyonu sonrası başağrısı yakınması oldu ve dura delinmesi sonrası başağrısı tanısı ile tedaviye başlandı. Tedaviye yanıt alınamaması üzerine radyolojik görüntüleme tetkikleri uygulandı ve sonuç olarak subdural hematom ve transvers sinus trombozu saptandı. Bu olgu sunumunda, anestezistlerin, spinal anestezi sonrası dura delinmesi sonrası başağrısı tanısı koyarken, ciddi bir durum olan subdural hematom ve serebral ven trombozunu da akılda tutmaları gerektiği güncel literatürler eşliğinde incelenerek bildirilmiştir.
