Nakil sonrası diabetes mellitus

Öz

Nakil sonrası diabetes mellitus (DM), solid organ nakilleri sonrasında sık rastlanan ve ciddi bir komplikasyondur. Böbrek nakil alıcılarında %4-40 oranında nakil sonrası DM oluştuğu bildirilmiştir. Yıllar içerisinde kullanılan farklı sınıflandırma sistemleri ve tanımlamalardan dolayı böbrek nakli sonrası gelişen nakil sonrası DM'nin gerçek insidansını belirlemek zordur. Nakil sonrası DM gelişen böbrek nakil alıcılarında fatal ve nonfatal kardiyovasküler olaylar ve enfeksiyon, azalmış hasta sağkalımı, graft rejeksiyonu, akselere greft fonksiyon kayıpları gibi diğer yan etkiler diyabet gelişmeyenlere göre daha sık gözlenmektedir. Erken tanı ve tanı sonrası nakil sonrası DM’ye yönelik özel antihiperglisemik tedavinin uygulanması, glisemik kontrol için gerekli olup nakil sonrası mortalite ve morbiditenin anlamlı ölçüde azalmasına katkı sağlamaktadır. Bu derlemede nakil sonrası DM’nin güncel tanısı, solid organ nakli sonrası prevalansı, potansiyel patogenetik mekanizmalar, risk faktörleri ve güncel tedavi yöntemleri literatür eşliğinde sunulmuştur.
