POSTTRAVMATİK DİYAFRAGMATİK HERNİYE SEKONDER GELİŞEN GASTROPLEVRAL FİSTÜL

Öz

Travmatik diyafragma hernisi tanısı gerek akut gerek kronik fazda, ayırıcı tanıda düşünülmediği sürece gözden kaçmaktadır. Toraksa nafiz bir kesicidelici alet yaralanmasından 2 yıl sonra hidropnömotoraks nedeniyle kliniğimize kabul edilen olguda gastroplevral fistül saptanarak cerrahi onarımı başarıyla yapılmıştır. Travmatik diyafragma hernisi sonrası gelişen gastroplevral fistül son derece nadirdir. Araştırmamıza göre daha önce travmatik diyafragma hernisini takiben ampiyem ve pnömotoraksla ortaya çıkan başka olgu bildirilmemiştir.

GASTROPLEURAL FISTULA AS A SECONDARY TO POSTTRAUMATIC DIAPHRAGMATIC HERNIA

Abstract

Traumatic diaphragmatic hernia diagnosis can not consider at the acut or chronic phase when in the distinctive diagnosis do not suggest. We report on a patient who applied to our clinic for hydropneumothorax 2 year after penetrating thoracic trauma, diagnosed as gastropleural fistula and surgical treatment was successfully done. The presentation of a traumatic diaphragmatic hernia as a gastropleural fistula has been rarely reported. To our knowledge, empyema with pneumothorax secondary to gastropleural fistula following a traumatic diaphragmatic rupture has not been described previously.
