AKCİĞERİN PRİMER TÜKRÜK BEZİ BENZERİ TÜMÖRÜ OLGUSU

Öz

Akciğerin primer tükrük bezi benzeri tümörü oldukça nadir görülür. Bu tip tümörler submukozal bronşial bezlerden orijin alırlar. Çoğunlukla ana bronşta lokalizedirler. Komplet rezeksiyon önerilen tedavi şeklidir. Adjuvan radyoterapi veya kemoterapi verilebilir. Kırkdört yaşında erkek olgu sağ omuz ve sırt ağrısı, hemoptizi ve nefes darlığı yakınmaları ile başvurdu. Posteroanterior akciğer grafisinde sağ akciğer üst zonda homojen dansite artışı izlendi. Toraks bilgisayarlı tomografisinde sağ apekste mediasten ve ana vasküler yapılara invaze tümöral lezyon ve bilateral multipl hematojen metastaz ile uyumlu lezyonlar saptandı. Bronkoskopide trakea alt uçtan başlayan sağ ana bronş girişinde submukozal tümöral infiltrasyon ve sağ üst lob bronş lümenini daraltan kitle lezyonu izlendi. Histopatolojk ve immunohistokimyasal inceleme sonucu akciğerin tükrük bezi benzeri tümörü olarak rapor edildi. Uzak metastazı saptanmayan ve Evre 4 olarak evrelendirilen olguya cisplatin bazlı kemoterapi verildi. Olgumuz nadir görülmesi nedeniyle literatür bilgileri eşliğinde sunulmuştur.

A CASE OF SALIVARY GLAND TYPE TUMOR OF LUNG

Öz

Salivary gland type tumor primarily arising in the lung is extremely rare. These types of tumors originate from submucosal bronchial glands. Most of them are located within the main bronchus. Complete surgical resection is recommended for treatment of this tumor usually followed by adjuvant radiation or chemotherapy. The patient is a 44-years-old man applied with complaints of right shoulder and right sided back pain, hemoptysis and shortness of breath. There was homogenius lesion in right upper lobe on posterior-anterior chest radiography. The right upper lobe contained a tumor mass extending into the large vessels and the mediastinum on thorax computerized tomography. Bilateral multipl pulmonary paranchymal metastasis were identified radiographically. In bronchoscopy, tumoral lesion that is starting from the proximal portion of the trakea and causing submucosal tumoral infiltration in right main bronchus and narrowing the lumen of right upper lobe bronchus was seen. Primary salivary gland type tumor of lung was reported in histopathological and immuno¬histochemical examination of biopsy that was taken from this regions. There were no distant metastasis. The case was accepted as stage 4 and received cisplatin based chemotheraphy. The case was presented with the review of literature as a rare entity.
