EVERY LYTIC BONE LESION IS NOT A METASTASIS: PRIMARY LUNG CANCER AND SYNCHRONOUS FIBROMATOSIS

Abstract

Bones are the frequent sites of metastases that are detected throughout the course of bronchial carcinoma. In this article, we present a case of a primary lung cancer in advanced stage and a lytic chest wall mass that had been initially suspected as bone metastasis. Mediastinoscopic biopsy showed an N2 lymph node and surgical biopsy from the chest wall lesion revealed fibro-matosis. As a result of chemotherapy, the fibro-matosis at the chest wall was also dissappeared as well, in addition to the lung cancer. With this report we would like to emphasize that not all the bone lesions although supported radiologically are metastatic and they shall undergo histo-pathological examination and in case of inoperable or irresectable fibromatosis, chemotherapy could be the choice of treatment.

HER LİTİK KEMİK LEZYONU METASTAZ DEĞİLDİR: PRİMER AKCİĞER KANSERİYLE EŞ ZAMANLI FİBROMATOZİS

Öz

Kemikler bronşiyal karsinomun sık metastaz yaptığı dokulardır. Bu çalışmada, tanı anında lokal ileri evre primer akciğer karsinomu ve başlangıçta litik kemik metastazı kuşkusu uyandıran göğüs duvarı kitlesi bulunan bir olgu sunulmuştur. Mediastinoskopi ile N2 hastalığı gösterilen olguya göğüs duvarı lezyonundan yapılan biyopsinin sonucunun fibromatozis olarak rapor edilmesi üzerine küratif kemotrapi ve mediastinal radyoterapi uygulanmış ve primer tümörde tam yanıt alınmıştır. Yine aynı tedaviyle göğüs duvarındaki fibromatoziste de belirgin regresyon izlenmiştir. Bu olguyla vurgulamak istediğimiz iki temel nokta, radyolojik olarak metastatik olduğu düşünülen kemik lezyonlarında, tedavi planını değiştireceği için eğer lezyonun yeri uygunsa histopatolojik tanı konulması gerektiği ve inoperabl veya irrezektabl olgularda fibromatozisin kemoterapi ile de tedavi edilebileceğidir.
