TÜBERKÜLOZA BAĞLI GELİŞEN TANSİYON HİDROPNÖMOTORAKS OLGUSU

Öz

Tüberküloz gelişmekte olan ülkelerde ve ülkemizde hala önemli bir sağlık problemi olup pnömotoraks ve hidropnömotoraks gibi komplikasyonlara yol açabilmektedir. Tansiyon pnömotoraks ise sıklıkla travma, mekanik ventilasyon ve spontan pnömotoraks sonrası gelişmektedir. Hastanemiz acil servisine ciddi tansiyon pnömotoraks tablosu ile başvuran 22 yaşındaki bir erkek hasta literatür verileri eşliğinde değerlendirildi. Hasta ani gelişen nefes darlığı, göğüs ağrısı şikayeti ile başvurdu, hipotansif, taşipneik, taşikardik ve siyanotik olduğu görüldü. Fizik muayenesinde solda solunum sesleri alınamadı. Akciğer grafisinde trakeada sağa deviasyon ve solda total pnömotoraks mevcuttu. Bilgisayarlı toraks tomografisinde solda hidropnömotoraks ve sol apikal bölgede kavitasyon izlendi. Balgam aside dirençli basil pozitif olarak saptandı. Literatür tarandığında tüberküloza bağlı hidropn ömotoraks sık gözlenmekle birlikte tüberküloza bağlı gelişen tansiyon hidropnömotoraks olgusuna rastlanmadı. İlk olgu olması nedeni ile sunmayı uygun bulduk.

A CASE WITH TENSION HYDROPNEUMOTHORAX DUE TO TUBERCULOSİS

Abstract

Tuberculosis is still one of the most important health problem in developing countries and in our country and can cause pneumothorax and hydropneumo-thorax as a complication. Tension pneumothorax frequently occurs after trauma, mechanic ventilation and spontaneous pneumothorax. 22 year old male patient with severe tension pneumothorax was evaulated with literature knowledge in our hospital emergency service. Patient entered with sudden onset shortness of breath and chest pain He was tachypnoeic, tachycardic and hypotensive. On physical examination no sound was heard on left lung. Tracheal right deviation and total pneumothorax on left was seen on a chest radiograph. Computerized thorax tomography revealed hydropneumothorax and apical cavitation.on left lung. A positive smear for acid-fast bacilli was found in sputum culture. Although hydropneumothorax due to tuberculosis can be seen often, we could find no reports of tension hydropneumothorax due to tuberculosis in the literature search. We presented this case as a first.
