PLEURAL AND BRONCHIAL METASTASIS OF COLON ADENOCARCINOMA: CASE REPORT

Although pulmonary metastases of extra pulmonary solid malign tumors are widely seen, endobronchial invasion has rarely been reported. Breast carcinoma, renal carcinoma, and colon adenocarcinoma are tumors, which can be result in endobronchial metastasis. A 74-year-old male patient referred to our clinic with complaints of dry cough, which is resistant to symptomatic treatment, continuing for 2,5 months and damaged his life quality. In his background, he had diagnosis of colon adenocarcinoma a year before, and history of 10 cures of chemotherapy application. Since pleural effusion was determined on the right through at the pulmonary graphy, he was hospitalized in our clinic. Malign epithelial cells were established in the pleural effusion obtained by torasynthesis and colon adenocarcinoma metastasis were determined in the pleura biopsy. On the bronchoscopy, the mucosa was diffusely edematous and hyperemic. Colon adenocarcinoma metastasis was showed in the mucosa biopsy.

KOLON ADENOKARSİNOMUNUN PLEVRAL VE BRONŞİYAL METASTAZI: OLGU SUNUMU

Öz

Akciğer dışı solid malign tümörlerin akciğer metastazlar ı yaygın olarak görülmesine rağmen endobron şiyal invazyon nadir olarak rapor edilmiştir. Meme karsinomu, renal karsinom ve kolon adenokarsinomu endobronşiyal metastazla sonuçlanabilen tümörlerdir. Yetmiş dört yaşında bir erkek hasta 2.5 aydır devam eden, yaşam kalitesini bozan, semptomatik tedaviye dirençli kuru öksürük şikayeti ile başvurdu. Özgeçmişinde; bir yıl önce kolon adenokarsinomu tanısı ve 10 kür kemoterapi uygulanma öyküsü mevcuttu. Hastanın çekilen akciğer grafisinde sağda plevral efüzyon saptanmas ı üzerine kliniğimize yatırıldı. Yapılan torasentez ile alınan plevral sıvıda malign epitelyal hücreler, plevra biyopsisinde ise kolon adenokarsinom metastazı saptandı. Hastanın yapılan bronkoskopide mukoza diffüz olarak ödemli ve hiperemikti, alınan mukoza biyopsisinde kolon adenokarsinom metastazı gösterildi.
