NADİR BİR EOZİNOFİLİ NEDENİ: ASPERGİLLOMA

Öz

Periferik eozinofili ve akciğerde infiltrasyonun bulunması eozinofilik akciğer hastalıkları başlığı altında incelenir. Atopi, parazitler, ilaçlar, vaskülitler başta olmak üzere bir çok sebep eozinofili yapar. Aspergillomanın eozinofiliye sebep olması nadir bir durumdur. Bu olgu sunumunda hemogramda eozinofili, yüksek çözünürlüklü bilgisayarlı tomografide infiltrasyon, bronşektazi ve kaviter lezyon ile aspergilloma saptanan otuz yedi yaşında erkek hasta sunuldu. Yapılan ayrıntılı incelemelerde eozinofili yapan herhangi bir neden bulunamadı. Tanı aşamasında eozinofilik akciğer hastalığı ayırıcı tanısına giren hastalıklar için ayrı ayrı değerlendirildi. Aspergilloma tedavisi olarak yapılan lobektomi neticesinde eozinofilinin düzeldiği gözlendi. Bu olgu eozinofilik akciğer hastalığı ayırıcı tanısında nadir de olsa aspergillomanın bulunması gerektiğini belirtmek amacıyla sunuldu.

ASPERGILLOMA: A RARE CAUSE OF EOSINOPHILIA

Abstract

Pulmonary infiltrates which associate with eosinophilia are known as ‘eosinophilic lung disease'. It occurs in many conditions such as athopia, parasitoses, drugs, vasculitis. Eoshinophilia of aspergilloma is a rare condition. We presented a case of aspergilloma. The patient was male and 37 years old. He has bronchiectasis, cavitary lesion and fungus ball and pulmonary infiltrates on HRCT scans; and blood eosinophilia. We could not find any cause for blood eosinophilia further investigations. After lobectomy blood eoshinophilia disappeared.
