HAND-HELD ULTRASONOGRAPHY DEVICE: IS REALLY A GOOD TOOL FOR CHEST PHYSICIANS?

Abstract

Aim: In recent years, use of ultrasonography becomes widespread increasingly in diagnosis of pulmonary and pleural diseases. The aim is to investigate the availability of hand-held ultrasonography that has been become available recently, at the bedside monitoring in the pulmonary diseases. Material and Methods: Hospitalized patients having indication for ultrasonography were included successively. Thoracic ultrasonography was performed by General Electric(GE)Vscan handheld ultrasonography with 1.7-3.8-MHz sector probe.When indicated thoracentesis or needle aspiration biopsy from the mass were performed. In cases where hand-held US is not sufficient, ultrasonography procedure was repeated with GE Logic 7 ultrasound and 3.5-MHz convex probe. Results: Hundred cases(44 male;56 female) with mean age of 64.0±16.4 years were included. In 80% pleural effusion, 14% mass lesions, 8% pericardial effusion, 6% consolidation was detected. In all of these cases(100%) sonographic findings detected with hand-held ultrasonography were correct. In 56cases having pleural fluid, thoracentesis under the guidance of hand-held US was performed at bedside.Transthoracic needle aspiration biopsy was performed in the companion of the hand-held US to the 4(28.6%) of 14 cases in which ultrasonographic findings consistent with mass in lung. No complications were seen in any of cases in which intervention was performed. Conclusion: The use of hand-held US is an effective method for the detection of thoracic pathologies,primarily for detection of pleural fluid, at the cases in which mobilization is difficult, hospitalized particularly in emergency room and intensive care unit.In the presence of the pleural fluid, the success of thoracentesis regardless the amount of the fluid is high and the method is secure.

YATAK BAŞINDA UYGULANAN AVUÇ İÇİ TORASİK ULTRASONOGRAFİNİN TANISAL DEĞERİ

Öz

Amaç: Son yıllarda, pulmoner ve plevral hastalıkların tanısında ultrasonografinin kullanımı giderek yaygınlaşmaktadır. Çalışmamızın amacı yeni kullanıma giren avuç içi ultrasonografi cihazının göğüs hastalıklarında yatak başında kullanılabilirliğini araştırmaktır. Yöntem ve Gereç: Çalışmaya, hastanemiz göğüs hastalıkları ve diğer kliniklerinde yatarak tetkik edilen, ultrasonografi endikasyonu konulan ardışık 100 hasta alındı. Torasik ultrasonografi, General Electric(GE) Vscan avuç içi ultrasonografi cihazı ile 1.7-3.8 MHz sektör prob kullanılarak uygulandı. Gerekli görülen olgularda torasentez veya kitleden iğne aspirasyon biyopsisi yapıldı. Avuç içi cihazın yetersiz kaldığı durumlarda GE Logic 7 ultrason cihazı ve 3,5 MHz konveks prob ile ultrasonografi tekrarlandı. Bulgular: Yaş ortalaması 64±16.4 yıl olan, 44 (%44)'ü kadın;56(%56)'i erkek toplam 100 olgu çalışmaya dahil edildi. Olguların %80'inde plevral efüzyon mevcuttu. Sekiz olguda perikardial sıvı saptanırken, 14 olguda kitle, 6 olguda ise konsolidasyon görüldü. Olguların tümünde(%100) avuç içi ultrasonografi ile saptanan bulgular doğru idi. Plevral sıvısı olan 56 olguda avuç içi USG rehberliğinde yatak başında torasentez yapıldı. Akciğerde kitle ile uyumlu sonografik bulgular saptanan 14 olgunun 4(%28.6)'üne ise avuç içi ultrasonografi eşliğinde transtorasik iğne aspirasyonu yapıldı. Kitle/nodül ile uyumlu lezyon saptanan diğer 10(%71.4) olguda ise biyopsi sırasında görüntü rezolüsyonunun optimal düzeyde olmaması nedeniyle biyopsi GE Logic 7 ultrason cihazı rehberliğinde alındı.Girişim yapılan olguların hiçbirinde komplikasyon görülmedi. Sonuç: Özellikle acil,yoğun bakım ünitesi ve servislerde yatan mobilizasyonu zor olan olgularda avuç içi ultrasonografinin kullanımı,başta plevral sıvı olmak üzere torasik patolojilerin saptanması için etkin bir yöntemdir.Plevral sıvı varlığında sıvının miktarına bakılmaksızın torasentez başarısı yüksektir ve yöntem güvenlidir. Kitle ve nodüler lezyonlar için transtorasik girişim sırasında avuçiçi USG cihazının yetersiz kaldığı durumlarda compakt cihazlar kullanılmalıdır.
