IS SURGERY ALWAYS NECESSARY FOR TRACHEAL LACERATION: CASE REPORT

Abstract

Although tracheal laceration due to entubation is a frequent complication, it might be morbid and mortal without a good management. Therefore early diagnosis and treatment is important. A 46 year old female was operated for an extrathoracic disease. She was investigated with chest computed tomography (CT) because of emphysema after extubation at her face and neck. A chest tube was inserted and removed at the 3rd day. After her follow up of 1 week, due to the lack of any regression of emphysema, she was referred to our department for futher investigation and management. As it is mentioned, although in case of early diagnosis the treatment of tracheal laceration is surgery, specifically to this late diagnosed patient the management of treatment is discussed under the light of the literature.

TRAKEA LASERASYONLARINDA HER ZAMAN CERRAHİ GEREKLİ Mİ?: OLGU SUNUMU

Öz

Entübasyona bağlı trakeal laserasyon nadir görülen bir komplikasyon olmakla birlikte, iyi yönetilemeyen olgularda morbid yada mortal seyredebilmektedir. Bu nedenle erken tanı ve tedavinin yönetimi önem taşımaktadır. Toraks dışı bir neden ile bir başka merkezde opere edilen 46 yaşında kadın hastada, ekstübasyon sonrası yüz ve boyunda cilt altı amfizemi gelişmesi üzerine, toraks bilgisayarlı tomografisi (BT) çekilmiş. Ardından sağ tüp torakostomi uygulanıp 3 gün sonra sonlandırılmış. Bir haftalık takibi sonrası cilt altı amfizeminin gerilememesi üzerine, ileri tetkik ve tedavi amacı ile göğüs cerrahisi kliniğimize sevki yapılmış. Belirtildiği gibi trakea laserasyonlarında erken tanı halinde uygulanacak tedavi acil cerrahi olsa da, tanısı gecikmiş olan bu olguya özel olarak uygulanan tedavi yönetimi literatür verileri eşliğinde tartışılmış ve sunulmuştur.
