Evaluation of Upper Gastrointestinal Subepithelial Lesions Using Radial Endosonography

Öz

Introduction: This study aims to present the results of radial endoscopic ultrasonography (EUS) used in the diagnostic evaluation of subepithelial lesions in the upper gastrointestinal (GI) system.
Materials and Methods: Patients who underwent examination with radial EUS for differential diagnosis of SELs in the upper GI tract were included. The character, echogenicity, size, originating layer, and diagnosis of the lesion from the EUS reports were recorded.
Results: Examinations with EUS found that lesions were more frequently observed in female patients (68.4% of all lesions were in women). Among the most common diagnoses for lesions were gastrointestinal stromal tumors (GIST) with 64 cases (26.2%), leiomyoma with 32 cases (13.5%), and lipoma with 24 cases (9.8%). When evaluating all lesions, the antrum was identified as the most common site of lesion occurrence. In terms of localization frequency, GISTs were most frequently observed in the esophagus, fundus, corpus, and bulb, leiomyomas were most commonly detected in the cardia, and inflammatory changes were predominantly found in the antrum.

Conclusion: Radial EUS stands out as a valuable tool in the diagnosis of subepithelial lesions in the upper GI tract, offering significant clinical advantages in the early detection and management of lesions with malignant potential, such as GISTs.

Radial Endosonografi ile Üst Gastrointestinal Sistem Subepitelyal Lezyonlarının Değerlendirmesi

Öz

Giriş: Bu çalışma, üst gastrointestinal sistemdeki subepitelyal lezyonların tanısal değerlendirilmesinde kullanılan radial endoskopik ultrasonografi (EUS) sonuçlarını sunmayı amaçlamaktadır.
Gereç ve Yöntem: Üst GİS’teki SEL’lerin ayırıcı tanısı için Radial EUS ile inceleme yapılan hastalar dahil edilmiştir. EUS raporlarında lezyonun karakteri, ekojenitesi, boyutu, hangi katmandan kaynaklandığı ve EUS ile tanısı kaydedilmiştir.
Bulgular: EUS ile yapılan incelemelerde kadın hastalarda lezyonların daha sık görüldüğü tespit edilmiştir (tüm lezyonların %68,4’ü kadınlarda). Lezyonların en yaygın tanıları arasında gastrointestinal stromal tümörler (GİST) 64 (%26,2), leiomyom 32 (%13,5) ve lipom 24 (%9,8) bulunmuştur. Tüm lezyonlara bakıldığında en çok antrumda lezyon tespit edilmiş, lokalizasyon açısından, özofagus, fundus, korpus ve bulbusta en çok GİST, kardiada en çok leiomyom, antrumda ise inflamatuvar değişiklikler tespit edilmiştir.
Sonuç: Radial EUS, üst gastrointestinal sistemdeki subepitelyal lezyonların tanısında değerli bir araç olarak öne çıkmakta, GİST gibi malign potansiyele sahip lezyonların erken teşhisi ve yönetiminde önemli klinik avantajlar sunmaktadır.
