POSTMENOPOZAL KANAMALI OLGULARIN ENDOBRUSH ENDOMETRİAL ÖRNEKLEME YÖNTEMİYLE DEĞERLENDİRİLMESİ

Öz

Bu araştırmada; postmenopozal kanamalı toplam 100 hastanın, endobrush sitolojik örnekleyiciyle elde edilen endometrial örnekleri, konvansiyonel probe küretaj ve histerektomi sonrası elde edilen sonuçlarla karşılaştırılarak endobrush tekniğinin kullanılabilirliği ve güvenilirliği çalışılmıştır. Endobrush yöntemiyle; yetersiz meteryal olarak 6 vaka, benign endometrium 65 vaka, kuşkulu sitoloji 9 vaka, bening hiperplazi 1 vaka, atipik hiperplazi 2 vaka ve malign sitoloji 17 vaka saptanmıştır. Histerektomi sonrası yapılan histolojik tanılar ile karşılaştırıldığında, endobrush tekniğinin kanser ve prekanseröz lezyonları saptamadaki duyarlılığı sensitivite %79.17 olarak, etkilenmemiş popülasyonu tespit etmedeki etkinliği ise spesifisite %85.71 olarak saptandı. Pozitif prediktif değer %65.52, negatif prediktif değer ise %92.31 bulundu. Endobrush gibi yeni endometrial örnekleyicilerin, postmenopozal kanamalı olgularda, malign endometrial patolojilerin erken tanısı ve izlenimi yanında, endometrium kanseri risk faktörü olanlarda tarama yöntemi olarak da kullanılmasının faydalı olacağı kanısına varıldı

EVALUATION OF THE ENDOMETRIUM WITH BRUSH SAMPLING IN WOMEN WITH POSTMENOPAUSAL BLEEDING

Öz

In this study, 100 women with postmenopausal bleeding were evaluated using the endobrush cytologic sampler and the results were compared with the hystological results of conventional curettage and hysterectomy. The sensitivity of cytological diagnosis of endometrial cancer and preinvasive endometrial lesions with endobrush evaluation was 79.17%, the specificity was 85.71%,and the positive predictive value was 65.52%, whereas the negative predictive value was as high as 92.31%. We concluded that endometrial sampling using the endobrush is a useful method in cytological detection of uterine lesions and in women with high risk of endometrial cancer
