BEKLENMEYEN LOKALİZASYONDA SAFRA TAŞI İLEUSU: OLGU SUNUMU

Öz

Safrataşı ileusu, safra taşının safra kesesinden barsaklara yer değiştirmesiyle oluşan mekanik intestinal obstruksiyondur. Kolelithiazisin nadir komplikasyonlarından biridir. 68 yaşında kadın hasta 3 gün önce başlayan karın ağrısı ve kusma şikayetleriyle acil servise başvurdu. Fizik muayenede batında her kadranda yaygın hassasiyet mevcuttu, defans ve rebaund yoktu. Beyaz küre sayısı 13100/mm3 idi. Ayakta direk batın grafisinde ince barsak düzeyinde hava-sıvı seviyeleri mevcuttu. Batın US’ de ince barsak anslarında dilatasyon tesbit edildi. Batın BT'de ise safra taşı ileusu ve periduodenal bölgede inflamatuar değişiklikler rapor edildi. Üst batın MR ve MRCP'de safra taşı ileusu, intrahepatik ve ekstrahepatik safra yollarında dilatasyon, safra kesesi perforasyonu ile uyumlu bulgular rapor edildi. Yapılan eksplorasyonda Treitz’dan itibaren 170 cm distalde jejenum düzeyinde lümeni tıkayan sert kitle tespit edildi. Enterotomi sonrasında yaklaşık 3x3cm boyutlarında taş çıkarıldı. Hastada komplikasyon gelişmedi ve operasyon sonrası 7. günde taburcu edildi. Sonuç olarak, safra taşı ileusu mekanik barsak obstrüksiyonlarının nadir sebeplerinden biridir. Obstrüksiyon genellikle ileoçekal valv ya da terminal ileumda görülmesine rağmen bizim olgumuzdaki gibi nadir de olsa jejenum düzeyinde de obstrüksiyon oluşabilir. Düşük riskli hastada tek aşamalı cerrahi, diğer hastalarda ise enterolitotomi uygulanması gereken cerrahi yöntemdir

GALLSTONE ILEUS IN UNEXPECTED LOCALIZATION: A CASE REPORT

Abstract

Gallstone ileus is a mechanical intestinal obstruction caused by migration of gallstone from gallbladder to intestine. It is one of the rare complication of cholelithiasis. 68 years old female patient admitted to emergency department complaining of abdominal pain and vomiting which started 3 days ago. There was abdominal rigidity on each quadrant and there wasn’t defence and rebaund on physical examination. White blood cell count was 13,100/mm3. Directly abdominal radiography showed small bowel air-fluid levels. Abdominal ultrasound showed small bowel dilatation. Abdominal CT revealed gallstone ileus and inflammatory changes in periduodenal region. MRI and MRCP revealed gallstone ileus, dilatation of intrahepatic and extrahepatic bile ducts and gallbladder perforation. Solid mass which obstructed the lumen of the jejenum 170 cm distal from Treitz was detected on exploration. Approximately 3X3 cm in size stone was removed from distal part of mass by enterotomy. Any complication no occurred on postoperative period. The patient was discharged 7th day after operation. In conclusion; gallstone ileus is one of the rare complication of mechanical bowel obstruction. Although obstruction is usually in terminal ileum or ileocecal valve, obstruction can be rarely occur at the level of jejenum as in our case. The single stage surgery should be performed in patients who have no additional risk factor. Enterolitotomy should be performed in others
