B 12 VİTAMİN EKSİKLİĞİNİN NÖROLOJİK KOMPLİKASYONLARINA DAİR BİR OLGU SUNUMU

Öz

Beş ay önce kolelitiyazis nedeniyle operasyon öyküsü olan otuz dokuz yaşında erkek hasta yürümede güçlük, denge bozukluğu şikayetleri ile kliniğimize başvurdu. B12 vitamin düzeyi düşük saptandı. Servikal torakal vertebra manyetik rezonans görüntülemede; subakut kombine dejenerasyon ile uyumlu sinyal artışı izlendi. Elektromiyografide sensoriyel polinöropati saptandı. Vizüel ve somatosensoriyel evoked potansiyellerde total ileti bloğu gözlendi. Hastaya parenteral olarak B12 vitamin replasmanı verildi. Tedavi sonrası klinik bulgularda düzelme gözlendi. Bu olgu, B12 vitamin eksikliğine bağlı elektrofizyolojik çalışmalar ve nörogörüntülemelerle kanıtlanmış optik nöropati, miyelopati, periferik nöropati ile giden nöroanemik sendrom olması nedeniyle tartışmaya uygun bulduk

THE RISK FACTORS FOR MALIGNANCY IN THYROID NODULES;HOW SHOULD THESE RISK FACTORS AFFECT THE DECISION FOR SURGERY?

Öz

Thirty nine years old male patient having cholelithiasis operation five months ago complaining walking difficulty, balance problems has been admitted to our hospital. Vit B12 level was found low. Subacute combined degeneration findings including increased signal intensity in cervical and thoracal magnetic resonance imaging were detected. In electromyography sensorial polyneuropathy was found. Visual and somatosensorial evoked potentials were all blocked. Parenteral Vit B12 replation therapy was given to the patient. After treatment, clinical findings were improved. Because this case has got neuroanemic syndrome proven with optic neuropathy, myelopathy, peripheral neuropathy with electrophysiology and neuroimaging techniques, we prefered to discuss here
