Paklitaksel Alan Meme Kanserli Kadınlarda Rahatsız Edici Semptomlar Teorisi'ne Dayalı Yapılan Yürüyüş Egzersizinin Periferal Nöropati ve Artralji-Miyalji Üzerine Etkisi: Randomize Kontrollü Pilot Uygulama Çalışma Protokolü

Öz

Amaç: Paklitaksel alan meme kanserli kadınlarda Rahatsız Edici Semptomlar Teorisi'ne dayalı yapılan yürüyüş egzersizinin periferal nöropati ve artralji-miyalji üzerine etkisini belirlemek amacıyla gerçekleştirilmiştir.
Gereç ve Yöntem: Araştırma, bir hastanesinin tıbbi onkoloji polikliniğinin ayaktan tedavi ünitesinde Temmuz 2022- Temmuz 2023 tarihleri arasında gerçekleştirilmiştir. Prospektif, tek kör, paralel grup randomize kontrollü tasarım olarak yapılmıştır. Örnekleme, blok randomizasyon yöntemi kullanılarak müdahale, kontrol grubu olmak üzere toplam 82 hasta dahil edilmiştir. Müdahale grubuna Rahatsız Edici Semptomlar Teorisi’ne göre fiziksel aktivitenin yararları konusunda bilgilendirmeler yapılmış, pedometreler verilmiş, günde ortalama 30 dakika yürümeleri ve adım sayılarını kaydetmeleri istenmiştir. Kontrol grubuna herhangi bir girişim yapılmamıştır. Veri toplama aracı olarak, Bilgi Formu, Kemoterapiye Bağlı Periferal Nöropati Değerlendirme Aracı ve McGill Ağrı Ölçeği Kısa Formu kullanılmıştır. Başlangıç, 4.8. ve 12. Haftalarda ölçekler uygulanmıştır. Çalışmaya katılan hastaların periferal nöropati ve artralji-miyaljiyi semptomları değerlendirilmiştir.Verilerin analizinde sayı, yüzde, ortalama, standart sapma, medyan, minimum, maksimum ve kategorik değişkenler için Pearson kikare/Fisher exact test, gruplar arası karşılaştırmada bağımsız gruplarda t testi, tekrarlı ölçümlerde ANOVA, grup-zaman etkileşimini değerlendirmek için karışık düzen ANOVA kullanılmıştır. Anlamlılık düzeyi p˂0,05 olarak kabul edilmiştir. Raporlamada CONSORT 2022 kullanılmıştır.
Sonuç: Çalışma yürüyüş egzersizinin paklitaksel alan meme kanserli kadınlarda rahatsız edici semptomlardan periferal nöropati ve artralji-miyaljiyi azaltmada yararlı olabileceği öngörülmüştür.

The Effect of Walking Exercise Based on The Theory of Disturbing Symptoms in Women with Breast Cancer Receiving Paclitaxel on Peripheral Neuropathy and Arthralgia-Myalgia: Randomized Control Pilot Study Protocol

Öz

Aim: This study was conducted to determine the effect of walking exercise based on the Theory of Disturbing Symptoms on peripheral neuropathy and arthralgia-myalgia in women with breast cancer receiving paclitaxel.
Materials and Methods: The study was conducted in the outpatient unit of the medical oncology outpatient clinic of a hospital between July 2022 and July 2023. It was conducted as a prospective, single-blind, parallel group randomized controlled design. A total of 82 patients were included in the sample as intervention and control groups using the block randomization method. The intervention group was informed about the benefits of physical activity according to the Disturbing Symptoms Theory, given pedometers, and asked to walk for an average of 30 minutes a day and record the number of steps. No intervention was performed in the control group. Information Form, Chemotherapy Induced Peripheral Neuropathy Assessment Tool and McGill Pain Scale short form were used as data collection tools. The scales were administered at baseline, weeks 4, 8 and 12. Symptoms of peripheral neuropathy and arthralgia-myalgia were evaluated. Number, percentage, mean, standard deviation, median, minimum, maximum, and Pearson chi-square/Fisher exact test for categorical variables, t test in independent groups for intergroup comparison, ANOVA for repeated measures, and mixed-order ANOVA to evaluate group-time interaction were used in the analysis of the data. Significance level was accepted as p˂0.05. CONSORT 2022 was used for reporting.
Conclusion: The study suggests that walking exercise may be beneficial in reducing the uncomfortable symptoms of peripheral neuropathy and arthralgia-myalgia in women with breast cancer receiving paclitaxel.
