Introduction: Typically, full arch reconstruction of edentulous ridges requires five to 10 dental
implants; however, some patients demanding fixed implant-supported prostheses are not able to medically or
economically afford complex bone grafts and/or a greater number of implants. These situations could pivot the
treatment plan toward the All-on-4 protocol.
Case Report: A 65-years old female patient presented our clinic in with an edentulous maxilla and
mandible. The All-on-4 protocol was selected for full mouth reconstruction. Four implants (Dentsply Ankylos,
Mannheim, Germany) were inserted in the maxilla and mandible. Three months later, radiographic evaluations
confirmed that the implants had successfully osseointegrated, so the prosthetic phase was initiated 2 weeks
after the placement of the healing caps. Next, diagnostic cast evaluation was performed. Because of the high
vertical distance between the maxillary and mandibular arches, it was decided to fabricate a modified hybrid
prosthesis to rehabilitate hard and soft tissue at the same time.
Discussion & Conclusion: A recent shift in practice paradigm has been to minimize treatment costs
and patient morbidity while providing the most satisfying patient-centered treatment outcomes according to
the state of the art of dental practice. The “All-on-Four” treatment concept is an attempt to reach these
objectives by providing relatively straight forward, predictable treatment option to rehabilitate edentulous
patients with a high outcome of quality of life.
This clinical report described the diagnosis, treatment planning, surgical treatment, prosthodontic
treatment, maintenance, complications, and a 6-months follow‐up of a patient who underwent treatment for a
full-mouth rehabilitation with all-on-4 fixed implant-supported hybrid prostheses. In the present case a hybrid
prosthesis has produced a satisfactory outcome from both the esthetic and functional points of view.
