Relationship Between Dermatological Involvement in Ulcerative Colitis and Serum Activin-A Levels

Background: Ulcerative colitis (UC) is a chronic inflammatory bowel disease characterized by intestinal and extraintestinal manifestations, including dermatological involvement. Activin-A, a member of the TGF-β superfamily, plays a critical role in inflammation and immune regulation. However, its relationship with dermatologic manifestations in UC remains unclear. Aim: This study aimed to investigate the relationship between dermatologic involvement and serum Activin-A levels in patients with active UC. Methods: A single-center observational study was conducted with 34 patients with active UC. Dermatological evaluations and serum Activin-A measurements were performed prior to immunosuppressive therapy. Data were analyzed using descriptive statistics, correlation, and chi-square tests. Results: Dermatologic manifestations were present in 13 (38.2%) of 34 patients. Median serum Activin-A levels were 516.34 pg/mL in patients with dermatologic involvement and 538.33 pg/mL in those without, with no statistically significant difference (p=0.807). No significant correlation was found between Activin-A levels and disease extent (Montreal E), severity (Montreal S), or other demographic variables. Conclusion: Dermatologic involvement in UC does not appear to be significantly associated with serum Activin-A levels. These findings suggest that Activin-A alone may not serve as a reliable systemic biomarker for skin manifestations in UC, highlighting the need for broader biomarker panels in clinical assessment.
