Predictive Value of Preoperative Neutrophil-to-Lymphocyte Ratio on Postoperative Complications in Valve Surgery Patients

Öz
Aim: It aims to retrospectively evaluate the relationship between preoperative neutrophil-lymphocyte ratio (NLR) values and the incidence of postoperative complications in patients undergoing heart valve replacement.
Material and Methods: This retrospective cohort study examined the data of 412 adult patients undergoing elective aortic and/or mitral valve replacement. NLR was calculated from blood samples 24 hours before surgery. Patients were divided into two groups based on NLR values: Group 1: NLR < 3.0 and Group 2: NLR ≥ 3.0.
Results: According to preoperative NLR values, 268 patients (65%) were in Group 1 and 144 patients (35%) were in Group 2. Significantly higher rates of postoperative atrial fibrillation (PoAF), renal dysfunction, prolonged mechanical ventilation, and infectious complications were observed in Group 2 compared to Group 1.
Conclusion: It demonstrates that preoperative NLR may be a simple and effective tool for risk stratification in patients undergoing valve surgery.

This retrospective cohort study was conducted in accordance with the Declaration of Helsinki Principles at the Department of Cardiovascular Surgery, Adana City Training and Research Hospital, Health Sciences University, and institutional ethical and legal permissions were obtained. Ethics committee approval was obtained from the Clinical Research Ethics Committee of Adana City Training and Research Hospital, Health Sciences University (Date: 21.08.2025 - Approval number: 16/689). The data used in this study were obtained from the hospital record system. The data were anonymized, and patient confidentiality was protected throughout the study. Informed consent was not required as there was no risk or impact on patient care. This waiver of consent was approved by the Institutional Review Board and the Ethics Committee.

Öz
Giriş: Kalp kapak replasmanı geçiren hastalarda preoperatif nötrofil-lenfosit oranı (NLR) değerleri ile postoperatif komplikasyon insidansı arasındaki ilişkiyi retrospektif olarak değerlendirmeyi amaçlamaktadır.
Gereç ve Yöntemler: Bu retrospektif kohort çalışmada elektif aort ve/veya mitral kapak replasmanı geçiren toplam 412 yetişkin hastanın verileri incelendi. NLR, ameliyattan 24 saat önceki kan örneklerinden hesaplandı. Hastalar NLR değerlerine göre iki gruba ayrıldı: Grup 1: NLR < 3,0 ve Grup 2: NLR ≥ 3,0.
Sonuç: Preoperatif NLR değerlerine göre 268 hasta (%65) Grup 1'de, 144 hasta (%35) Grup 2'de yer aldı. Grup 2'de, Grup 1'e kıyasla anlamlı derecede daha yüksek oranda postoperatif atriyal fibrilasyon (PoAF), böbrek fonksiyon bozukluğu, uzun süreli mekanik ventilasyon ve enfeksiyöz komplikasyonlar görüldü.
Tartışma: Preoperatif NLR'nin kapak cerrahisi geçiren hastalarda risk sınıflandırması için basit ve etkili bir araç olabileceğini göstermektedir.

Bu retrospektif kohort çalışma, Helsinki Prensipleri Deklarasyonu'na uygun olarak Sağlık Bilimleri Üniversitesi Adana Şehir Eğitim ve Araştırma Hastanesi Kalp Damar Cerrahisi Anabilim Dalı'nda yürütülmüş ve kurumsal etik ve yasal izinler alınmıştır. Etik kurul onayı Sağlık Bilimleri Üniversitesi Adana Şehir Eğitim ve Araştırma Hastanesi Klinik Araştırmalar Etik Kurulu'ndan alınmıştır (Tarih: 21.08.2025 - Onay numarası: 16/689). Bu çalışmada kullanılan veriler hastane kayıt sisteminden elde edilmiştir. Veriler anonimleştirilmiş ve çalışma boyunca hasta gizliliği korunmuştur. Hasta bakımı üzerinde herhangi bir risk veya etki olmadığı için aydınlatılmış onam gerekmemiştir. Bu onam feragati Kurumsal İnceleme Kurulu ve Etik Kurul tarafından onaylanmıştır.
