Comparison of the Therapeutic Efficacy of Antibiotic Therapy, Nasal Steroids, Isotonic Saline, and Hypertonic Saline in Patients with Acute Rhinosinusitis

Purpose: Acute rhinosinusitis (ARS) is a common upper respiratory tract disorder characterized by inflammation of the nasal and paranasal sinus mucosa. Many factors play a significant role in its etiology, particularly viruses. The optimal treatment approach remains controversial. The aim of this study was to investigate the effects of various treatment methods on sinusitis symptoms in patients with ARS. Material and Methods: Patients were divided into five groups: antibiotic therapy, intranasal steroid, nasal irrigation with isotonic saline, nasal irrigation with hypertonic saline, and a control group. Symptoms were assessed and compared using a Visual Analog Scale (VAS) before and after treatment. All evaluations were performed by a physician blinded to the treatment groups. Results: The differences between pre- and post-treatment VAS scores were statistically significant within all treatment groups. However, the change in the control group was not significant. Furthermore, the post-treatment VAS scores of patients who received antibiotic therapy were significantly better than those of the other groups. Conclusion: Antibiotic therapy appears to be the most effective treatment for acute rhinosinusitis.

Akut Rinosinüzitli Hastalarda Antibiyotik Tedavisi, Nazal Steroid, İzotonik Serum ve Hipertonik Serumun Tedavi Etkinliğinin Karşılaştırılması

Öz

Amaç: Akut rinosinüzit (ARS), nazal ve paranazal sinüs mukozasının inflamasyonu ile karakterize yaygın bir üst solunum yolu hastalığıdır. Etiyolojisinde özellikle virüsler olmak üzere birçok faktör önemli rol oynamaktadır. En uygun tedavi yaklaşımı ise hâlâ tartışmalıdır. Bu çalışmanın amacı, ARS’li hastalarda çeşitli tedavi yöntemlerinin sinüzit semptomları üzerindeki etkilerini araştırmaktır. Materyal ve Metot: Hastalar beş gruba ayrıldı: antibiyotik tedavisi, intranazal steroid, izotonik serum ile nazal irrigasyon, hipertonik serum ile nazal irrigasyon ve kontrol grubu. Semptomlar tedavi öncesi ve sonrası Görsel Analog Skala (VAS) kullanılarak değerlendirildi ve karşılaştırıldı. Tüm değerlendirmeler, tedavi gruplarına kör olan bir hekim tarafından yapıldı. Bulgular: Tüm tedavi gruplarında tedavi öncesi ve sonrası VAS skorları arasındaki farklar istatistiksel olarak anlamlı bulundu. Ancak kontrol grubundaki değişim anlamlı değildi. Ayrıca antibiyotik tedavisi alan hastaların tedavi sonrası VAS skorları, diğer gruplara kıyasla anlamlı derecede daha iyi saptandı. Sonuç: Antibiyotik tedavisi, akut rinosinüzit için en etkili tedavi yöntemi olarak görünmektedir.
