BİR ÜÇÜNCÜ BASAMAK YOĞUN BAKIM ÜNİTESİNDE TAKİP EDİLEN ERİŞKİN TRAFİK KAZASI HASTALARININ KLİNİK ÖZELLİKLERİ VE SONUÇLARI

Öz

ÖZET Amaç: Bu çalışmada, 3. basamak bir yoğun bakım ünitesinde trafik kazası nedeniyle tedavi gören yetişkin hastaların klinik özelliklerini, aldıkları tedavileri ve mortalite ile ilişkili faktörleri saptamayı amaçladık. Gereç ve Yöntem: 2020–2022 yılları arasında trafik kazası nedeniyle Anestezi Yoğun Bakım Ünitesi’ne yatırılan 18 yaş ve üzeri 183 hasta retrospektif olarak incelendi. Yaş, cinsiyet, kaza türü, yatış Glasgow Koma Skoru (GKS), APACHE II ve TRISS skorları, cerrahi müdahale durumu, laboratuvar bulguları, yoğun bakımda kalış süresi ve çıkış durumları değerlendirildi. Mortalitenin bağımsız belirleyicileri lojistik regresyon analizi ile incelendi. Bulgular: Hastaların 153’ü (%83,6) erkek, 30’u (%16,4) kadındı. Ölen hastaların oranı %11,5 (21 hasta) olarak saptandı. Ölen hastalarda sağ kalan hastalara göre Hb değeri, GKS, TRISS skoru, daha düşüktü; laktat, APACHE II skoru, kan transfüzyonu ihtiyacı ve inotropik ajan ihtiyacı daha yüksekti. Lojistik regresyon testinde TRISS skorunun mortalite için bağımsız risk faktörü olduğu gösterildi. Sonuç: Trafik kazaları yoğun bakımdaki önemli mortalite nedenlerindendir. TRISS skoru bu hastalarda mortaliteyi öngörmek için güçlü bir göstergedir. Sonuç: Trafik kazaları yoğun bakımdaki önemli mortalite nedenlerindendir. Clinical Characteristics and Outcomes of Adult Traffic Accident Patients in a Tertiary Care Intensive Care Unit

Öz

ABSTRACT Objective: This study aimed to evaluate the clinical characteristics, treatments administered and mortality-related factors of adult patients treated for traffic accident–related trauma in a tertiary care intensive care unit. Materials and Methods: A total of 183 patients aged ≥18 years who were admitted to the Anesthesia Intensive Care Unit between 2020 and 2022 due to traffic accidents were retrospectively analyzed. Demographic data, type of accident, admission Glasgow Coma Scale (GCS), APACHE II and TRISS scores, surgical intervention status, laboratory findings, length of ICU stay, and discharge outcomes were evaluated. Independent predictors of mortality were assessed using logistic regression analysis. Results: Of the patients, 153 (83.6%) were male and 30 (16.4%) were female. The overall mortality rate was 11.5% (21 patients). Compared to survivors, non-survivor patients had significantly lower hemoglobin levels, GCS scores, and TRISS scores, while lactate levels, APACHE II scores, need for blood transfusion, and requirement for inotropic support were significantly higher. Logistic regression analysis identified a low TRISS score as an independent risk factor for mortality. Conclusion: Traffic accidents represent an important cause of mortality in intensive care units. The TRISS score is a strong and independent predictor of mortality in this patient population. Key words: traffic accident, TRISS, Intensive care unit, Mortality
