Anestezi yönetiminde laringeal maske ve endotrakeal tüp kullanımının maliyet analizlerinin karşılaştırılması: prospektif çalışma

Öz

Amaç/Gerekçe: Bu çalışmanın temel amacı, 0-2 saat süren kısa süreli plastik cerrahi operasyonlarında havayolu yönetiminde yaygın olarak tercih edilen laringeal maske (LM) ve endotrakeal tüp (ET) uygulamalarının doğrudan maliyetlerini ve klinik sonuçlarını detaylı bir biçimde karşılaştırmaktır. Gereç ve Yöntemler: Prospektif gözlemsel olarak tasarlanan bu çalışmaya, 2018-2019 yılları arasında elektif plastik cerrahi operasyonu geçiren 18-65 yaş arası toplam 80 hasta dahil edilmiştir. Hastalar anestezi yönetimi için endotrakeal tüp (Grup ET, n=40) ve laringeal maske (Grup LM, n=40; LMA ProSeal™) olmak üzere iki gruba ayrılmıştır. Hastaların demografik verileri, entübasyon, ekstübasyon ve operasyon süreleri ile anestezi indüksiyonu ve idamesinde kullanılan tüm ilaçların dozları kaydedilmiştir. Her iki yöntemin doğrudan ilaç ve tıbbi malzeme maliyetleri hesaplanarak gruplar arasında karşılaştırılmıştır. Bulgular: Grup ET'de entübasyon (p<0.001), ekstübasyon (p=0.029) ve operasyon (p<0.001) süreleri Grup LM'ye göre istatistiksel olarak anlamlı derecede daha uzun bulunmuştur. İlaç tüketimi incelendiğinde; Grup ET'de remifentanil, roküronyum, atropin, neostigmin dozları ve kullanılan havayolu aracı sayısı daha yüksekken, Grup LM'de sadece propofol dozu daha yüksek saptanmıştır. Grup ET'de işlem sürelerinin uzamasına ve ilaç sarfiyatının fazla olmasına rağmen, ortalama toplam maliyet Grup LM'de anlamlı düzeyde daha yüksek bulunmuştur (LM: 173.59 TL, ET: 159.08 TL; p<0.001). Sonuç: Doğrudan ilaç ve malzeme maliyetleri analiz edildiğinde, endotrakeal tüp kullanımının laringeal maskeye kıyasla daha avantajlı ve düşük maliyetli bir alternatif olduğu saptanmıştır. Ancak, klinisyenler ve hastane yöneticileri tıbbi malzeme tercihi yaparken sadece maliyetleri değil; hasta konforunu, hemodinamik stabiliteyi ve postoperatif klinik sonuçları da kapsayan bütüncül bir değerlendirme yapmalıdır.

Comparison of cost analyses of laryngeal mask and endotracheal tube use in anaesthesia management: prospective study

Öz

Objective/Background: This study aimed to compare the direct costs and clinical outcomes of endotracheal tube (ET) and laryngeal mask (LM) applications, which are widely preferred for airway management during short-term plastic surgery operations lasting up to two hours. Materials and Methods: This prospective observational study, conducted between 2018 and 2019, included 80 patients aged 18-65 years undergoing elective plastic surgery. Participants were assigned to either the endotracheal tube (Group ET, n=40) or laryngeal mask group (Group LM, n=40; LMA ProSeal™). Demographic characteristics, procedure times (intubation, extubation, operation), and precise doses of all anesthesia drugs were recorded. Direct medication and medical supply costs for both methods were calculated and statistically compared. Results: Intubation (p<0.001), extubation (p=0.029), and operation (p<0.001) times were significantly longer in Group ET. Regarding drug consumption, the doses of remifentanil, rocuronium, atropine, neostigmine, and the number of airways used were higher in Group ET, whereas only propofol was higher in Group LM. Despite prolonged procedure times and increased drug consumption in Group ET, the mean total cost was significantly higher in Group LM (LM: 173.59 TL vs. ET: 159.08 TL; p<0.001). Conclusion: Based on direct material and medication costs, the endotracheal tube was found to be a more cost-effective alternative than the laryngeal mask. However, when selecting medical supplies, clinicians must not rely solely on costs but should holistically evaluate patient comfort, hemodynamic stability, and postoperative clinical outcomes.
