Primary Gastric Extraosseous Osteosarcoma with Extensive Multivisceral Invasion: A Case Report

Öz

Introduction: Extraosseous osteosarcoma (ESOS) is a rare soft-tissue sarcoma that produces osteoid without any anatomical relationship to the skeleton. Primary gastric ESOS is exceptionally rare and presents major clinical, radiological, and pathological diagnostic challenges. Case Presentation: A 78-year-old man with six months of nonspecific gastrointestinal symptoms was found to have a 14×16×21 cm gastric mass invading the left hepatic lobe, pancreas, duodenum, and omentum. Endoscopic ultrasound–guided fine-needle aspiration was non-diagnostic. After massive upper gastrointestinal hemorrhage with hemodynamic instability, life-saving emergency laparotomy with R2 debulking was performed. Histopathology and SATB2-positive immunohistochemistry confirmed high-grade extraosseous osteosarcoma. PET-CT showed no distant or skeletal lesion, supporting a primary gastric origin. Conclusion: Primary gastric ESOS may present as a giant, multivisceral mass with non-diagnostic biopsies. Awareness, multidisciplinary evaluation, and timely surgery are critical, as resectability remains the principal prognostic determinant. Keywords: Gastrointestinal Hemorrhage; Osteosarcoma; Sarcoma; Soft Tissue Neoplasms; Stomach Neoplasms.

We refer to our manuscript entitled “Primary Gastric Extraosseous Osteosarcoma with Extensive Multivisceral Invasion: A Case Report”, submitted for consideration as a Case Report in the Journal of Cukurova Anesthesia and Surgical Sciences. We hereby declare that, in accordance with the regulations and standard practice of Çukurova University Faculty of Medicine, institutional ethics committee approval is not required for the publication of an anonymized single-patient case report. The manuscript reports the diagnostic and therapeutic course of a single patient and does not constitute a research project involving prospective intervention, randomization, or systematic data collection from human subjects that would fall within the scope of mandatory ethics committee review. Notwithstanding the above, we confirm that the present case report has been prepared in full compliance with internationally accepted ethical principles. In particular: 1. All procedures performed in this case were in accordance with the ethical standards of our institution and with the principles of the 1964 Declaration of Helsinki and its later amendments, or comparable ethical standards. 2. Written informed consent for the publication of clinical details and accompanying images was obtained from the patient prior to manuscript submission. The signed consent form is retained in the patient’s medical record and is available for review by the editorial office upon request. 3. All patient identifiers have been removed from the manuscript, figures, and any supporting material. Imaging and histopathological figures have been anonymized and contain no information that would permit identification of the patient. 4. No experimental intervention was undertaken solely for the purposes of this report; all diagnostic and therapeutic procedures described were carried out as part of routine clinical care. Should the editorial board nevertheless require formal documentation from our institutional ethics committee, we will be pleased to obtain and submit such documentation without delay. Please do not hesitate to contact the corresponding author for any further information or clarification. We thank you for your kind consideration.
