İnterstisyel Akciğer Hastalığında Üst Ekstremite Kas Enduransının Klinik Yansımaları

Öz

Amaç: İnterstisyel akciğer hastalığı (İAH), akciğer interstisyumunu etkileyen kronik bir hastalık grubudur. Günlük yaşamda üst ekstremitenin önemli rolü düşünüldüğünde, üst ekstremite performansının klinik durumla ilişkisini belirlemek önemlidir. Çalışmamızda amacımız İAH olan bireylerde üst ekstremite enduransının yorgunluk, günlük yaşam aktiviteleri ve yaşam kalitesi arasındaki ilişkiyi araştırmaktır
Yöntem: İAH tanılı, kriterlere uygun 25 gönüllü değerlendirildi. Demografik veriler kaydedildi ve GAP indeksi hesaplandı. Üst ekstremite egzersiz enduransları Desteklenmeyen Üst Ekstremite Egzersiz Testi (UULEX), dispne ile Modifiye Medical Research Council Scale (MMRC), yorgunluk şiddetleri Yorgunluk Şiddet Ölçeği (FSS), günlük yaşam aktiviteleri London-Chest Günlük Yaşam Aktiviteleri Ölçeği (LCADL) ve yaşam kaliteleri St. George Solunum Anketi (SGRQ) kullanılarak değerlendirildi.
Bulgular: Katılımcıların yaş ortalaması 62,08±7,09 yıldı (Kadın/Erkek= %28/%72). Çoğu (%56) GAP Evre 1'deydi. UULEX test süresi MMRC (r:-0,671, p<0,01), FSS toplam skoru (r:-0,509, p<0,05), LCADL toplam skoru (r:-0,491, p<0,05) ve SGRQ toplam skoru (r:-0,499, p<0,05) arasında negatif yönlü, orta düzeyde ilişkiler bulundu
Sonuç: Üst ekstremite enduransı, İAH’ta dispne, yorgunluk, günlük yaşam aktiviteleri ve yaşam kalitesi ile ilişkili bulunmuştur. Bulgularımıza göre UULEX ile değerlendirilen Üst ekstremite enduransı stabil İAH ve eforla desatüre olan hastalarda değerli bir klinik değerlendirme aracı olabilir.

Clinical Reflections of Upper Extremity Muscle Endurance in Interstitial Lung Disease

Öz

Objective: Interstitial lung disease (ILD) is a group of chronic disorders affecting the lung interstitium. Considering the key role of the upper extremities in daily activities, determining their relationship with clinical status is important. This study aimed to investigate the relationship between upper extremity endurance and fatigue, daily living activities, and quality of life in individuals with ILD.
Methods: Twenty-five volunteers diagnosed with ILD and meeting the inclusion criteria were evaluated. Demographic data were recorded and GAP index scores calculated. Upper extremity endurance was assessed with the Unsupported Upper Limb Exercise Test (UULEX); dyspnea with the Modified Medical Research Council Scale (MMRC); fatigue with the Fatigue Severity Scale (FSS); daily living activities with the London Chest Activity of Daily Living Scale (LCADL); and quality of life with the St. George’s Respiratory Questionnaire (SGRQ).
Results: The mean age of participants was 62.08 ± 7.09 years (Female/Male = 28%/72%). Most (56%) were in GAP Stage 1. Moderate negative correlations were found between UULEX duration and MMRC (r = –0.671, p < 0.01), total FSS (r = –0.509, p < 0.05), total LCADL (r = –0.491, p < 0.05), and total SGRQ (r = –0.499, p < 0.05).
Conclusion: Upper extremity endurance was found to be associated with dyspnea, fatigue, activities of daily living, and quality of life in ILD. According to our findings, upper extremity endurance assessed with the UULEX may serve as a valuable clinical assessment tool in stable ILD and in patients who desaturate with exertion.
