Ortopedik Bakımdaki Mükemmeliyet Merkezleri: Sağlık Yönetimi Perspektifinden Uzmanlaşmış Ortopedi Hastanelerinin Karşılaştırmalı Anlatı İncelemesi

Öz

Bu anlatı inceleme, ortopedik bakımda mükemmeliyet merkezlerini (CoE) sağlık yönetimi perspektifinden ele alarak, New York'taki Hospital for Special Surgery (HSS), Berlin'deki Charité, Bologna'daki Istituto Ortopedico Rizzoli ve İstanbul'daki Baltalimanı Kemik Hastalıkları Eğitim ve Araştırma Hastanesi'ni karşılaştırmalı olarak analiz etmektedir. Klinik sonuçlar yerine yönetim modelleri, organizasyon yapıları ve stratejik uygulamalara odaklanarak, CoE'lerin uzmanlaşma, hacim-kalite ilişkisi, inovasyon ve sürdürülebilirlik dinamiklerini inceler. Araştırma, ikincil kaynaklar (resmi raporlar, akademik yayınlar) ve benchmarking yöntemiyle yürütülmüştür. Bulgular, akademik (HSS, Charité) ve vakıf (Rizzoli) modellerinin inovasyon odaklı hibrit yönetişimi, Baltalimanı'nın kamu odaklı erişilebilirlik stratejilerini öne çıkarır. Dijital dönüşüm (AI ve robotik cerrahi), değer temelli bakım, insan kaynakları ve uluslararası hasta yönetimi gibi boyutlarda benzerlikler ve farklılıklar tablolarla sunulmuştur. Sonuçlar, hastane yöneticilerine hibrit modeller için çıkarımlar sunar; politika yapıcılara ise küresel adaptasyon yol haritası çizer. Sınırlılıklar arasında ikincil verilerin önyargısı ve klinik derinlik eksikliği yer alır; gelecek araştırmalar AI ROI'si ve eşitlik denetimlerini önerir. Centers of Excellence in Orthopedic Care: A Comparative Review of Specialized Hospitals from a Healthcare Management Perspective

Öz

This narrative review examines centers of excellence (CoEs) in orthopedic care from a healthcare management perspective, conducting a comparative analysis of the Hospital for Special Surgery (HSS) in New York, Charité in Berlin, Istituto Ortopedico Rizzoli in Bologna, and Baltalimanı Bone Diseases Training and Research Hospital in Istanbul. Prioritizing management models, organizational structures, and strategic practices over clinical outcomes, it explores the dynamics of specialization, volume-quality linkages, innovation, and sustainability. Employing secondary sources (official reports, academic publications) and benchmarking, the study reveals hybrid governance in academic (HSS, Charité) and foundational (Rizzoli) models fostering innovation, contrasted with Baltalimanı's public-focused accessibility strategies. Dimensions such as digital transformation (AI and robotic surgery), value-based care, human resources, and international patient management are delineated through similarities and differences in tables. Implications offer hospital managers insights for hybrid models and policymakers a global adaptation roadmap. Limitations include secondary data biases and clinical depth exclusions; future research proposes AI ROI and equity audits. Orthopedic CoEs emerge as pioneers of resilient health systems through managerial excellence.
