Diz Şikayeti İle Kliniğe Başvuran Olgularda, Alt Ekstremite Fonksiyonu, Denge ve Günlük Yaşam Aktivitelerinin Değerlendirilmesi

Öz

Çalışmamız bireylerde en çok görülen diz patolojilerinin, denge ve propriosepsiyon, fonksiyonellik, günlük yaşam aktiviteleri ve yaşam kalitesi üzerine etkisini değerlendirmeyi amaçlamaktadır. 30-65 yaş arasındaki 304 hasta, aldıkları tanılara göre osteoartrit (OA), menisküs yırtığı (MY) ve ön-arka çapraz bağ (ÖÇB-AÇB) grupları olacak şekilde 3 gruba ayrıldı. Ağrı ve günlük yaşam aktiviteleri değerlendirmesi için “Diz Yaralanma ve Osteoartrit Sonuç Skoru” ve “Günlük Yaşam Uğraşlarına İlişkin Diz Testi”, denge ve propriosepsiyon değerlendirmesi için “Berg Denge Ölçeği” ve “Tek Ayak Üzerinde Durma Testi”, fonksiyonel yetenekler değerlendirmesi için “Zamanlı Kalk Yürü Testi”, “5 Defa Otur Kalk Testi” ve “Fonksiyonel Uzanma Testi” web tabanlı sistemler ile online randevu oluşturulup uygulandı. Demografik veriler kişisel bilgi formu ile toplanmıştır. Tüm değerlendirmelerde gruplar arasında istatistiksel olarak anlamlı düzeyde fark bulunamamıştır (p>0,05). Her üç grup içi değişkenler arasındaki ilişkilere bakıldığında; KOOS-Diz tüm alt başlıkları arasında istatistiksel olarak anlamlı düzeyde ilişki, BDÖ ile fonksiyonel yetenek testlerinin skorları arasında istatistiksel anlamlı ilişki, BDÖ ile KOS-ADLS skorları arasında anlamlı ilişki bulunmuştur (p<0,05). Günlük yaşam aktiviteleri (GYA) ve fonksiyonel yetenek testleri skorları arasındaki ilişkiye bakıldığında MY ve OA grubunda istatistiksel olarak anlamlı düzeyde ilişki saptanmıştır (p<0,05). ÖÇB-AÇB grubunun ağrı ve GYA skorları daha üstündü. Tüm gruplarda propriosepsiyon azalmış olup denge ve düşme riski artmıştır. Dinamik dengenin sağlanması ve sürdürülmesi tüm patolojilerde zorlaşmıştır. Çalışma verilerinin literatür ile karşılaştırıldığında web tabanlı sistemler üzerinden bireylerin değerlendirmelerinin sağlık çalışanları tarafından yapılabileceğini göstermektedir. Geliştirilebilecek yazılımların değerlendirme içeriği olarak dinamik ve statik denge, propriosepsiyon, GYA ve fonksiyonel becerileri içeren ölçütleri içermelidir.

Evaluation Of Lower Extremity Function, Balance And Daily Life Activities In Patients Applied To The Clinic With Complaint Of Knee

Öz

The aim of this study was to evaluate the effects of the most common knee pathologies in individuals on balance and proprioception, functionality, activities of daily living and quality of life. In our study, 304 patients aged between 30 and 65 years old were divided into three groups according to their diagnoses; osteoarthritis (OA) group, meniscal tear (MT) group and anterior-posterior cruciate ligament (ACL-PCL) group. Pain and activities of daily living were evaluated with “Knee Injury and Osteoarthritis Outcome Score” (KOOS-KNEE) and “Knee Outcome Survey Activities of Daily Living Scale” (KOS-ADLS). Balance and proprioception were evaluated with “Berg Balance Scale” (BBS), “Single Leg Stand Test” (SLST) and also for the evaluation of functional abilities “Time Up and Go Test” (TUG), “Five Times Sit to Stant Test” (FTSTS), and “Functional Reach Test” (FRT) were used. There was no statistically significant difference in all evaluation between groups (p>0,05). There were statistically significant correlations in all subscales of KOOS-KNEE, between BBS scores and functional abilities measurement, between BBS and KOS-ADLS scores within groups (p<0,05). The statisttically significant correlations was found between Activity Daily living (ADL) and functional abilities measurements in OA and MT groups (p<0,05). Pain and ADL scores in the ACL-PCL group was better. Proprioception was decreased in all groups, and balance and risk of falling were increased. Establishing and maintaining dynamic balance has become difficult in all pathologies. When the study data is compared with the literature, it shows that the evaluation of individuals through web-based systems can be done by health professionals. The evaluation content of the software that can be developed should include criteria including dynamic and static balance, proprioception, ADL and functional skills.
