Monthly and Sex-Specific Suicide Mortality in Turkey: Implications for Intensive Care (2007-2024)

Objective: Suicide shows well-documented intra-annual variation, yet few studies have examined month-by-sex patterns over long observation windows or in Eastern Mediterranean populations. This study characterized the monthly distribution of suicide mortality in Turkey across 2007–2024 using population- and month-length-adjusted rates, and tested whether the seasonal pattern differs between men and women. Materials and Methods: This retrospective, observational study used publicly available aggregate data from the Turkish Statistical Institute report on death and cause-of-death statistics. Monthly suicide counts by sex for 2007 through 2024 yielded 432 observations. Quasi-Poisson generalized linear models (log link) were fitted with the natural logarithm of person-days of exposure (sex-specific mid year population × days in month) as an offset, adjusting for calendar year as a categorical covariate. The month-by-sex interaction was tested with Wald tests; monthly contrasts were Bonferroni-adjusted; sensitivity analyses covered 2009–2024, exclusion of the pandemic years 2020–2021, and month-by-period and month-by-COVID interactions. Results: Suicide rates showed significant monthly variation (p<0.001), rising from April, peaking in June (incidence rate ratio [IRR] versus December 1.22, 95% CI 1.15–1.30) with June and July statistically indistinguishable (Bonferroni p=1.000), and falling to a November–February trough. Rates were 2.86 fold higher in men than in women (95% CI 2.78–2.94). The month-by-sex interaction was not significant (Wald χ²(11)=11.90, p=0.371), and this null finding was unchanged in the 2009–2024 and COVID-excluded sensitivity analyses. Seasonal amplitude attenuated in 2016–2024 relative to 2007 2015 (interaction p=0.013) with unchanged peak timing. Conclusion: Suicide mortality in Turkey follows a consistent late-spring-to-summer peak (April August, maximal in June) and a November–February trough, similar in timing and shape in both sexes. These findings are hypothesis-generating for temporally informed health-service planning and warrant validation with hospital-level admission data.

Türkiye'de İntihar Mortalitesinin Aylık ve Cinsiyete Özgü Örüntüsü: Yoğun Bakım Çıkarımları (2007-2024)

Öz

Amaç: İntiharın yıl içinde belirli aylarda yoğunlaştığı iyi bilinmektedir; ancak aya ve cinsiyete göre örüntüyü uzun bir gözlem süresi boyunca inceleyen çalışmalar, özellikle Doğu Akdeniz toplumlarında sınırlıdır. Bu çalışma, Türkiye'de 2007–2024 döneminde nüfusa ve ay uzunluğuna göre düzeltilmiş aylık intihar hızlarını tanımlamayı ve mevsimsel örüntünün kadınlar ile erkekler arasında farklılık gösterip göstermediğini incelemeyi amaçlamıştır. Materyal ve Metot: Bu retrospektif, gözlemsel çalışmada, Türkiye İstatistik Kurumu'nun ölüm ve ölüm nedeni istatistiklerine ilişkin kamuya açık toplulaştırılmış verileri kullanılmıştır. 2007–2024 yıllarına ait cinsiyete göre aylık intihar sayıları 432 gözlem oluşturmuştur. Aşırı dağılım nedeniyle Pearson ölçek düzeltmeli quasi-Poisson genelleştirilmiş doğrusal model kullanılmış; kişi-gün cinsinden maruziyet (cinsiyete özgü yıl ortası nüfus × aydaki gün sayısı) logaritmik offset olarak eklenmiş, takvim yılı kategorik değişken olarak kontrol edilmiştir. Ay×cinsiyet etkileşimi Wald testiyle sınanmış; aylar Bonferroni düzeltmeli ikili karşılaştırmalarla incelenmiş; 2009–2024 dönemi, COVID-19 yıllarının (2020 2021) dışlanması ile ay×dönem ve ay×COVID etkileşimleri duyarlılık analizleri olarak yapılmıştır. Bulgular: Aylık intihar hızları anlamlı mevsimsel değişim göstermiştir (p<0.001): hızlar Nisan–Ağustos döneminde yükselmiş, en yüksek değere Haziran'da ulaşmış (Aralık'a göre insidans hız oranı [IRR]=1.22; %95 GA 1.15–1.30), Haziran ile Temmuz arasında fark saptanmamıştır (Bonferroni p=1.000). Erkeklerde intihar hızı kadınların 2.86 katıdır (%95 GA 2.78–2.94). Ay×cinsiyet etkileşimi anlamlı bulunmamış (p=0.371) ve bu sonuç 2009–2024 ile COVID hariç analizlerde değişmemiştir. Mevsimsel genlik 2016 2024 döneminde 2007–2015'e göre zayıflamış (p=0.013), zirve zamanlaması değişmemiştir. Sonuç: Türkiye'de intihar hızları, her iki cinsiyette benzer zamanlama ve biçimle, tutarlı bir geç ilkbahar yaz yükselişi (Nisan–Ağustos, en yüksek Haziran) ve Kasım–Şubat dip dönemi göstermektedir. Bu bulgular, zamansal olarak planlanmış sağlık hizmeti öngörüleri için hipotez üreten niteliktedir ve hastane düzeyindeki yatış verileriyle doğrulanmayı gerektirir.
